Abstract | Uric acid was measured in serum and urine of the ambulant and hospitalized patients with various disorders including gout. in Misasa-Branch Hospital of Okayama University Medical School from January 1975 to December 1976. Serum and urine samples were obtained from 283 males and 260 females, and from 2 males respectively. The results are as follws; (1) Uric acid in males was 1.4 mg/100mf higher than in females. (2) Uric acid in males decreased with ages, showing two peaks between 15 and19 , and between 35 and 44 years old. (3) Uric acid in females showed low levels between third and fifth decades and increased thereafter with ages. (4) There were many fatty subjects among the patients with gout. (5) An increase in total daily excretion of urinary uric acid was observed concomitantly with increased daily urine volume. (6) Periodical fluctuations of uric acid in serum and other clinical findings were show in several cases of gout, who had been controlled in MisasaBranch Hospital for several years. |

Abstract | Since Copeman's report on patients with hearing impairment as "rheumatoid otoarthritis?" has introduced, there have been a few reports on hearing impairment in patients with rheumatoid arthritis such as Goodwill et al. and Djupesland et aI. in European countries. On the contrary in our country there is no reference in association with hearing impairment of rheumatoid arthritis in rheumatic or otological literatures. An audiometric survey was carried out in 76 patients admitted in the Misasa branch hospital of Okayama university school of medicine on the classical or definite rheumatoid arthritis. Patients were excluded from the series if they had scarred or perforated tympanic membrane and history of otorrhea. Thus 67 patients have come to study. As control group, 15 healthy persons in hearing for each ten years, 15-24, 25-34, 35-44, 45-54, 55-64, and over 60 years, were selected and as a physiological hearing limit, rejection limit of hearingloss in dB. was made. Among 67 patients, 111 ears had hearingloss within the above mentioned rejection limit. In 23 ears, hearingloss in dB. were partial or as a whole out of the limit. They have all air-bone-conduction gap. After inflation of Eustachien tube, in most of them air conduction ability showed nearly the same level of bone conduction. In three cases even by inflation, airconduction level did not move. In one case the left side had effusion liquid and its RAreaction was positive. TwO of the former patients and the one whose effusion liquid had positive RA-test followed up for about 3 years. In the former two cases, hearing impairment got worse little by little and recovery by inflation was not seen. On both cases Gelle's test was positive. In the latter hearing impairment slowly got worse. This had lasted for about two months and had recoverd. In the se four cases heairng impairment was probably related to the rheumatoid arthritis and no other causes could be found. |

Abstract | A plasma emission spectrophotometer "Spectraspan" (low power d. c. plasma arc, operating on argon) was evaluated for boron determination in thermal waters. The influence of acids (hydrochloric, nitric and sulfuric acids) and several metallic ions (Na(+). K(+), Mg(2+). Ca(2+)) on emission intensity was studied. There was a linear relationship between emission intensity and boron content from 0 to 500 mg/l. Boron in thermal waters was easily determined by standard addition method. Precision. coefficient of variance and recoveries of known amount of boron added to the sample for 11 repricate analyses were 0.12μg/ml, 2.02%, 95.0-101.7%, respectively. Boron content of 27 thermal waters in the Sanin district was determined. and the highest B content in the sample waters were 8.8 mg/l (Tottori spa) in Tottori Prefecture and 14.6-25.0 mg/l (the thermal springs at the foot of Mt. Sanbe. a non-active quaternary volcano) in Shimane Prefecture. The relationships between B content and water temperature, B content and pH value were not recognized. Significantly positive correlationship was observed between Band Li contents. |

Abstract | Sinter deposit in a distributing pipe used for about fifteen years in Misasa radioactive hot springs was investigated. The results were as follows; 1. The metal elements in this deposit were quaIi. tatively analysed by spectraspan plasma emission spectrophotometry (SPES), by fluorescent X-ray spectrometry, and following elements were detected; B, Na, Mg, AI, K, Ca, Mn, Fe, Cu, Zn, As, Sr, Ba and Pb. 2. The metal elements in this deposit were quantatively analysed by SPES and atomic absorption spectrophotometry, and following elements were determined; Fe : 508.7, As: 26.40, Ca: 7.85, Mn : 2.00, Al : 1.80, Na: 1.60, Zn: 1.33, K: 0.80, Cu: 0.67, Sr: 0.47, Mg: 0.35, Ba: 0.33 and B : 0.30 mg per gram. The color of this deposit was red brown, and the main component was ferric oxide. 3. Radioactive elements in this deposit were detected by autoradiography and radioluxography. Among the radioactive elements, concentration of radium was 320.5×10(-12)g/g (313.0×10(-12)Ci/g). |

Abstract | Lithium in serum was determined by a plasma emissionspectrophotometer "Spectraspan" (low power d.c. plasma are, operating on argon). The opitimum condition for ashing of serum by low temperature was studied. The influences of hydrochloric acid and several metallic ions (Na, K, Mg, Ca) on emission intensity were examined. There was a linear relationship between emission intensity and lithium content from 0 to 10μg/ml. Lithum in serum was easily determined by standard addition method. Precision, coefficient of variance and recovery of known amount of lithium added to the sample for 12 replicate analyses were 0.093μg/ml, 1.85%, 99-104%, respectively. Significantly positive correlationship was observed between emission spectrophotometry using spectraspan and atomic absoption spectrophotometry. |

Abstract | N 13-b method for determination of uric acid by AutoAnalyzer- I was proposed from Technicon Corporation, but the sensitivity of this method was insufficient. According to the modification by Patel, good sensitivity was obtained for determining of uric acid, but insufficient separation between peaks was accompanied. To get good separation, Patel's method was reexamined by the authors, and the results were as follows. Sufficient separation was obtained by modifing the flow diagram of Patel's method. The mean recovery rates of uric acid added to serum and urine were 103.6% and 102.5%, and coefficients of variation were 1.27% and 1.05% respectively. The correlation between this modified Patel's method and the U.A. test Wako method was recognized (serum : n=95, r=0.97, urine : n=35, r =0.99). According to our modified method, uric acid concentration in serum of 68 subjects, were 5.8±0.9mg/100ml in 29 males and 4.4±0.8mg/100ml in 39 females. |

Abstract | An automatic blood analyser, Coulter Counter Model Sr(CC-SR) was recently introduced in our hospital. This blood analyser is able to provide simultaneous measurements of seven blood parameters, i.e. WBC, REC, Hgb, Hct, MCV, MCH, MCHC, and also produces the results within 40 seconds from aspiration of the sample into the instrument. The performance and precision of this blood analyser have been clitically evaluated by comparing the results obtained from the previous blood cell counter, the Coulter Counter Model Dn (CC-DN), a haemoglobinometer, and a high speed centfugal separator. The results are as follows: 1) The precision and liniarity for readings and dilutions on WBC, RBC, Hgb and Hct by the use of the CC-SR is satisfactory. 2) Good correlation between the measurements by whole blood analysis procedure and micro sample analysis procedure is recognized. 3) The amount of carryover from one sample to the next is negligible for practical purpose. 4) High correlation factors are obtained between the measurements by the CC-SR method and former methods: i. e. 0.99 for WBC, 0.93 for RBC, 0.99 for Hgb and 0.99 for Hct. 5) For these reason, it can be said that CC-SR is more useful successive instrument to measure WBC, RBC, Hgb, Hct than the CC-DN and other above mentioned instruments. |

Abstract | A combination therapy of ketamine-amtriptyline for the intractable pain with restlessness of cancer patients in the end stage was presented. Seven patients with peritonitis carcinomatosis due to recurrence of stomach cancer were administrated the therapy. They ranged in age from 29 to 79 years. The results were eminently desirable. Intractable pain with restlessness were diminished by the therapy though languid feeling was yet remained. Anodyne (pentazocine), which was nessesary many times in one day before the therapy, was not used in the duration of the therapy. The experimental and clinical studies of analgesic property of ketamine by HATANO revealed that it is more twice as potent an analgesic in low dosage compared with pethidine, despite its rather short duration of action. There was no respiratory depression. No hypotension nor bradycardia were observed during the therapy. The only use of ketamine however, could not release the pain and anguish of cancer patients. The author applied amitriptyline to these cases. The combination use of ketamine and amitriptyline could release them. These drugs demonstrated their full effect by the combination use. |

Abstract | It is thought that a drug reacts differently with media taken internally. The solubility of Aspirin is assured 600μg/ml in 30min. with Misasa spring water, whereas 400μg/ml with plain water. The parallel difference of 200μg/ml follows 10 min. thereafter. Maximum serum concentratin of 200μg/ml is obtained 2 hours after the internal use of Aspirin with Misasa spring water in parallel difference of 20μg/ml with plain water. In the case of Indomethacin intake, maximum serum concentration of 1.5μg/ml is obtained in 2 hours with plain water, whereas maximum serum concentration of 1μg/ml is obtained in the same period with Misasa spring water. However, the difference of 0.25μg/ml in serum concentration between the media internally taken is seen initially and after 4 hours under high concentration of Indomethacin. We, therefore, assume tentatively that the effect of Aspirin and Indomethacin is supposed to be enhanced with Misasa spring water taken internally. |

Abstract | The author reviewed 55 patients who had been treated as anorexia nervosa at the neuropsychiatric department of Okayama University Medical School and at the internal medicine department of Misasa Branch Hospital, Okayama University Medical School, for the past ten years from 1970 to 1980. The criteria for the present study was as follows; 1) absence of psychosis and no known physical illness for ematiation, 2) weight loss of at least 2096 of original body weight, 3) duration of anorexia of at least three months. 55 patients were classified into three groups according to the age of onset ; pre-puberty group. adolescent group and adult group. Analysis of two female adolescent cases was described in this report for the preliminary study. The results were as follows : 1) Onset of self-starvation coincided with competitious hyperactivity in sport and/or study. Self-starvation seemed to have psychological meaning of retaliation towards the mother of each patient, of compensatory gain in the dependancy need. and of keeping a pride in pubertal competitions including a pursuit in the slim body image. 2) There was the evidence of a disturbed female-identification which originated from the disturbed mother-child interaction. 3) Catharsis with analytically oriented psychotherapy improved gastro-intestinal disorders such as anorexia and constipation and reversed the negative BMR (basal metabolic rate) to the positive BMR. Recovery of menstrual periods was followed in an accordance with re-gain of the original body weight. |

Abstract | Presented here is a case report of a 56 years old male patient with chronic pancreatitis in which a dramatic improvement in symptoms and objective findings were noted after endoscopic elimination of protein plugs. In September, 1979, he had an acute onset of pancreatitis followed by pseudocyst formation and the following combined operations were performed: resection of the pseudocyst, resection of the body and tail of the pancreas along with the spleen, partial resection of the stomach and the transverse colon, and gastroenterostomy according to Billroth I method. He had been followed up at out-patient clinic for chronic pancreatitis and diabetes until January 14,1979 when he was hospitalized for anorexia, loss of weight, pretibial edema, and upper-abdominal pain. The patient's condition was characterized by emaciation (height 152 cm, weight 37 kg), anemia (Hb 7.9g/dl), hypoproteinemia (serum protein 4.2 g/dl) , and hypolipidemia (total cholesterol 73 mg/dl). Abdominal ultrasound, abdominal CT and ERCP revealed stones in the pancreatic duct. Symptoms and objective findings were only partially improved by medical regimens. Subsequently we performed endoscopic cannulation of the pancreatic duct for elimination of pancreatic stones. Successful elimination of large numbers of protein plugs resulted in dramatic improvement of symptoms and objective findings. This procedure proved to add a new important tool in the treatment of chronic pancreatitis. |

Abstract | Endoscopic aspiration of pure pancreatic juice (ppj) was performed on 65 patients with chronic pancreatitis (cp) or suspected CP primarily for evaluating the validity of biochemical and physiochemical analysis of PPJ in the study of pathogenesis, pathophysiology and diagnosis of CP. PPJ was collected from within the main pancreatic duct by endoscopic retrograde catheterization of the papilla. With increasing experience, it has been revealed that various numbers of protein plugs are contained in PPJ obteined from patients with CP or suspected CP and that endoscopic elimination of protein plugs can provide them with dramatic relief from abdominal pain and back pain. The purpose of this report is to evaluate the role of endoscopic elimination of protein plugs in the treatment of CP. The following conclusions were obteined: (1) endoscopic elimination of protein plugs from within the pancreatic duct is useful. at least on a short-term basis, for relieving abdominal pain and back pain in one-forth of patients with CP or suspected CP; (2) this procedure can be an effective therapeutic tool in selected cases of CP or suspected CP where no prominent stenotic lesions are noted in the major pancreatic duct system and abstinence from alcohol bevarage is strictly observed. |

Abstract | Adverse side effects of steroid therapy were investigated in 32 asthmatic patients. Thirty-two patients were classified into three groups according to steroid therapy for the past five years; group 1 has been treated with continuous steroid therapy, group 2 with occasional steroid therapy and group 3 without steroid therapy. The results were as follows. 1. Group 1 showed a low level of serum cortisol at 8-9 a. m. The serum concentration of cortisol in patients with daily steroid regimen was lower as compared to that in patients with alternateday steroid therapy. 2. The daily profile of serum cortisol was low in the steroid dependent asthmatic patients, and little increase of serum cortisol level after the administration of prednisolone was shown in group 1. 3. Serum IgG and IgM levels were significantly low in steroid dependent asthmatic patients. 4. The level of serum potassium was low in group 1. |

Abstract | Five severe bronchial asthma patients with glucocorticoid therapy have had free swimming training in the hot spring pool at Misasa Medical Branch. The clinical effect of the swimming training was evaluated after 3 months' training. 1. A slight increase in pulse rate and blood pressure was observed immediately after 30 minutes, swimming, although no significant increase in the two parameters was shown 30 min. after the training. 2. No significant fall in ventilatory function tests such as % FVC, FEV(1.0)%, V(50) and V(25) was demonstrated following the swimming training in a hot spring pool. The results suggest that the free swimming training in a hot spring pool does not induce bronchoconstriction. 3. Clinical efficacy of the swimming training on bronchial asthma was evaluated according to a treatment score, an attack score and an asthmatic score. These scores were decreased during the swimming training. The results obtained in this study showed that free swimming training in a hot spring pool is one of the most suitable therapies for severe bronchial asthma patients. |

Abstract | IgE-mediated release of histamine from whole blood was examined in two healthy and four asthmatic subjects by dose-dependent fashion. The significantly increased amount of histamine was released from basophils of both healthy and asthmatic subjects by a limited concentration of anti-IgE. Antigen (house dust) caused histamine release only from basophils of asthmatics who are sensitive to house dust. Basophils from one patients with asthma released no significant amount of histamine by anti-IgE. |

Abstract | Histamine released from whole blood was determined by an automated fiuorometric histamine analysis system. The increased release of histamine from basophils by anti-IgE was observed in ten healthy subjects and 12 extrinsic asthma patients, while the release in 11 intrinsic asthma patients was significantly less as compared to that in healthy and extrinsic asthma subjects. House dust extract caused a significant increase in the histamine release from basophils of the extrinsic asthma patients who are sensitive to house dust. It was concluded from this study that histamine released from basophils could be easily determined by an automated analysis system and that the method is useful for the diagnosis and study of allergy. |

Abstract | Following the previous report in which analysis of two female adolescent cases was described, two female adult cases were presented in this report. The summery as follows: 1) Onset of two cases is in their early twenties, coincided with their inter-personal conflicts at work and in the family in law. Morbid condition of each case has been unchanged todate, over the age of fourty; case 1 presents chronic anorexia with bulimia and case 2 presents chronic anorexia without bulimia. 2) There was the evidence of immaturity of heterosexual object-relationship which originated from the disturbed father-daughter interaction, together with the evidence of a disturbed female identification as seen in the adolescent group. 3) Self-starvation of each case appears to have similar psychological meaning as seen in the adolescent cases; retaliation towards the mother of each patient and compensatory aim in the dependancy need. However, keeping a pride in pubertal competitions such as a pursuit in the slim body image may shift to a phobic avoidance of gastro-intestinal dyscomfort. Eventully poor motivation for treatment aimed at tackling the adolescent maturational problems seems to be the core of the disorder. |

Abstract | The case histories of 5 males diagnosed as anorexia nervosa were presented. The summary as follows : (1) 2 pre-adolescent cases; Onset of self-starvation of each case is coincided with separation from each mother, with the psychological meaning of retaliation towards the mother of each patient and of compensatory gain in dependancy need. Rorschach test of either case presents the ambivalent attitude towards each mother, of love and hate. (2) An adolescent case; Onset of self-starvation is coincided with competitious hyperactivity in sport, with some compensatory gain in dependancy need and with keeping pride in pubertal competition, including a pursuit in the slim body image. Rorschach test presents pausity of masculinity. (3) A young case; Onset of self-starvation is coincided with separation from his mother, as seen in pre-adolescent case, with phobic avoidance of protenuria from which he suffered previously. Rorschach test indicates paucity of masculinity. (4) An adult case (married); Onset of self-starvation is coincided with his conflicts at work and home, with the psychological meaning of keeping him from undoing duties as a worker and as a husband. Rorschach test presents mother-identification instead of father-identification. |
