| Abstract | これまで､腎移植者のQOLの向上をめざした系統的アプローチを看護援助モデルを基に検討してきた｡そして現在､腎移植者に対する効果的かつ具体的な看護介入について検討を進めている｡本研究では､移植された腎臓と受けている医療に不満を抱く腎移植者6名の事例分析を基に､腎移植者への看護的関わりを検討した｡その結果､この6名の移植者が抱く不満は､身体状態､責任を果す能力と周囲のサポート､自己実現と人生観､仕事と経済的自立､家族との関係の5つのカテゴリーに分類された｡そして､6名に共通する不満は､身体状態の問題に起因した健康に対してであった｡また､生体腎移植者はすべて家族に対する役割や他人に対する社会的役割の達成について不満を抱いていた｡そして､男性の移植者は､周囲からのサポートに満足していなかった｡これらのことは､腎移植者のQOL向上に向けた看護介入を具体化する上で最も考慮すべき要点であることが明らかとなった｡ |

| Abstract | 本研究は,介護保険法の利用者と家族の満足度や,介護保険法がどのように受け止めているのかを明らかにすることを目的とした｡対象は,介護保険法の中で訪問看護を受けている利用者の家族の方(主に介護を担当されている方)95名で,郵送法により52名の回答を得た(回収率54.7%)｡研究期間は,2000年7月から9月であり,アンケートは介護にかかる費用･介護サービスの2点に重点をおいて作成した｡その結果,以下の実態を把握することができた｡ 1.介護保険法を利用するにあたり,介護サービスにおける費用の問題が多い｡介護サービスの内容には満足していたが,介護サービスにかかる費用は高いと感じている｡ 2.介護者は,介護をしていて｢利用者のこれからのことが心配である｣,｢自分の時間が持てない｣と感じている人が多いことがわかった｡ 3.介護保険法や利用契約の手続きが複雑であるという意見が多かった｡今後,介護保険法の利用に向けて,これらの問題点に対する解決への取り組みが必要であることが示唆された｡ |

| Abstract | 1986年のオタワ宣言以来,ヘルスプロモーションは先進工業国を中心に健康づくりの柱として健康政策策定に大きな役割を果たしてきた｡20世紀後半になるとアジア諸国を始めとする発展途上国でも人口の高齢化や慢性疾患の増加に対応するために,ヘルスプロモーションを視野に入れた健康政策が策定されるようになった｡このような状況の中で,各国国民のヘルスプロモーションに対する看護職が担うべき役割が世界的に問われている｡｢ヘルスプロモーションを通した発展:看護者が重要な成果を生み出す｣と題した国際会議がタイで開催され,共催校の一つであるミシガン大学看護学部の教授で,世界保健機関(WHO)とともに長年ヘルスプロモーションの研究に取り組んでいるベンダー教授の基調報告は,それに対する回答でもあった｡すなわち,看護者は科学者として成長するために,国際的共同研究の蓄積に努め,文化的背景が異なる人々にヘルスプロモーションの介入計画･評価を発展的に進めていくことが重要である｡ |

| Abstract | 2つの老人保健施設に入所している痴呆老人について,観察者による彼らの満足度とその時に観察された行動の関係を明らかにすることを目的とする｡調査対象者は,2つの老人保健施設に入所中で,本人または家族より調査への了系の得られた,歩行可能な痴呆老人各14名,合計28名である｡観察者は対象者の行動を48時間観察し,どこで誰と何を行っているかとその時の満足度を調査票に記録した｡満足度は対象者の表情や態度から観察者が判断し,大変満足を⑤とし,どちらとも言えないを③,大変不満足を①とした5段階評定で示した｡その結果以下の3点が明らかとなった｡ 1)両施設で満足度の高い時間帯は,レクリエーションが含まれる"午前中の時間"であった｡ 2)満足度が高いのは,居場所では,"レクリエーションルーム･屋外"であった｡ 3)同行者は,"家族","観察者",行動は,"話をする",手伝いをするなど"自発的な行為"が多くみられた｡以上により,2つの施設で満足度ごとの時間帯,居場所,同行者と行動が似通っていたことから,客観的に痴呆老人の満足度を把握することは可能であると考える｡ |

| Abstract | 食道癌患者は腫瘍による狭窄のため,程度の差はあるものの,術前より通過障害を認めることが多い｡食道癌と診断され,手術を自己決定した患者の多くは,手術をして罹患前のように食事ができるようになりたいと思っている｡しかし,患者の多くが術後に嚥下感覚の変化に遭遇する｡現在,手術前に実施されるインフォームドコンセント(以下IC)では,術後に起こる嚥下感覚の変化については説明が行われていない｡そのため患者は,経口摂取が可能になった時点で嚥下感覚の変化を自覚し,強い不安を感じる｡今回, 5人の患者に術後の嚥下感覚の変化に関するインタビューを実施するとともに,術後の嚥下活動を頸部電気インピーダンスおよび嚥下音を用いて測定した｡インタビューの結果, 5人全てが,術後に嚥下感覚の変化を自覚し,不安を感じていた｡また,青年健常者と比較し,IPG波形の多相化と,嚥下音の咽頭期以外での発生が認められた｡患者が,術後に起こる嚥下感覚の変化についてのICを望んでいることからも,術後の嚥下感覚の変化についてのICを行なう必要がある｡ |

| Abstract | 地域で生活する高齢者･障害者にとって,住宅環境の整備は必須の要件である｡津山市では在宅生活を支援する立場にある保健婦やホームヘルパー,作業療法士等保健･福祉関係者と,建築関係者が共同で支援チームをつくり住宅改造に関わってきた｡本研究では,この支援チームが関わった事例と直接関わらないで当事者と業者のみで改造を実施した事例を比較し,支援チームが関わることの効果をみた｡調査方法は,各事例の家庭を訪問して面接聞き取り調査を行った｡調査内容は調査票を用いて改造箇所,生活空間,介護量,外出の状況について尋ねた｡改造前後の変化を比較して,支援チームの効果について以下のことが明らかになった｡ 1.支援チームの関与による浴室の改造は,入浴が可能となり生活空間が広がるなど改善が認められた｡ 2.離床を目的にした玄関･廊下･居室の改造は,外出などの日常生活行動を広げ,本人のQOLを高めることに役立つ｡以上より住宅の改造が,介護者の介護負担を必ずしも直接軽減するとは限らないが,高齢者･障害者の介護の質に変化を与えることが示唆された。 |

| Abstract | 腎移植は,レシピエントのQOLを高めることで期待される治療法であるが,移植後の拒絶反応や合併症,あるいはそれらに伴う心理社会的問題などによって移植後のQOLに影響を及ぼす危険性を孕んでいる｡本研究では,腎移植後レシピエントのQOL向上を目指した看護援助を検討するために,移植後のQOLに影響すると考えられる移植を受けた理由やそのときの気持ち,ローカスオブコントロールをとりあげた。本研究の目的は,そのようなレシピエントの背景がQOLとどのように関係しているのかを明らかにすることである｡対象者は研究の同意を得た腎移植後のレシピエント119名であり,人生に対する感じ方,移植を受けた理由やそのときの気持ち,ローカスオブコントロール,QOLなどについて自己報告調査を行った｡その結果,レシピエントは移植後に何らかの身体症状や合併症が生じていても,81%の者が人生を肯定的に感じていた｡そして,生活を充実させたいとして移植を受けた者が,また,ローカスオブコントロールの内的統制傾向が強い者ほどQOLを高める傾向にあった｡したがって,看護婦はQOLに影響を及ぼすレシピエントの背景を加味して,移植前から適切な看護援助を行う必要性が示唆された｡ |

| Abstract | 病院におけるアメニティの重要性は十数年前から言われているが,それは建物の建築の時だけでなく,その後療養の場所をどのように維持し,快適環境を患者にいつまでも提供していくかである｡これまで日常の看護業務において掲示物やパンフレットの置き方,床頭台のあり方などは整理整頓の一環で病棟管理の中にあった｡だが,もう一つの流れがある｡ウイリアム・モリスは生活の中における芸術化を考え,生活用品そのものに美しきと手作りの良さがあることを提唱した｡こうした生活デザインの流れの中で,本研究では本学科棟内において床頭台のディスプレイや掲示の仕方,パンフレットの置き方など,ラッピング技法を使用し,入院生活上のアメニティの創出を試みた｡その後,ラッピング技法を用いたアメニティ創出の試みは患者の心を癒す可能性のあることを明らかにした｡また,ラッピング技法使用上の留意点についても指摘した｡ |

| Abstract | 本研究では慢性腎疾患を有する若い成人の内的経験世界を検討した｡5名の患者が疾病に関してどのようなことを経験しているのかについて非組織的な深層面接を受けた｡分析の結果,患者の内的経験世界は,(1)患者が経験している感情や思考,(2)疾病や自己についての解釈,(3)重要な他者への信頼,(4)自己の変化と発達(成長)という4つに特徴づけられることが明らかになった｡しかるべき質の高い看護を提供するためには,看護者は,患者の内的経験世界を理解することが必要である｡ |

| Abstract | 吸虫Spelotrema capellaeの飼い猫への感染源を調べるために,広島県福山市を中心とした瀬戸内沿岸の河口域に生息するカニにおけるメタセルカリアの感染状況を調査した｡調査地のうち広島児内の4ヵ所と岡山県内の1ヵ所のケフサイソガニHemigrapsus PenicillatusからS. capellaeのメタセルカリアが検出された｡特に,過去に症例報告のあった猫の行動圏内にある福山市の2ヵ所のケフサイソガニからは高率に検出された｡したがってこれらの地方においては飼い猫へのS. capellaeの感染源は河口産のケフサイソガニであることが強く示唆された｡ |

| Abstract | 本総説は,本邦で実施している三朝ラドン温泉の適応症に関する機構解明に資するために調査した欧州におけるラドン療法の医学的研究に関する最近の動向の概要についてまとめたものである｡すなわち,ラドンを用いた温泉や坑道での療法の適応症には,脊椎の非細菌性炎症(べヒテレフ病),慢性多発性関節炎,気管支喘息などが含まれる｡臨床医学的研究として,以前,客観的方法で検討されたものはほとんどなかったが,最近,無作為化二重盲検臨床試験などにより,ラドン療法がベヒテレフ病,慢性多発性関節炎などの患者の痛みを緩和させる効能があることが実証されつつある｡また,基礎医学的研究として,抗酸化機能や免疫機能などの指標に着目した動物実験により,ラドン療法の機構を解明する上で合理的根拠が得られつつあることもわかった｡さらに,これらの知見と本邦での研究動向を踏まえ,今後,期待されるラドン療法の機構解明を行うための研究課題を提案した｡ |

| Abstract | Six species of Paragonimus are presently known in Latin America. They include the following : P. mexicanus, P. peruvianus, P. amazonicus, P. inca, P. ecuadoriensis, and P. caliensis. Among them, P. peruvianus has been recognized as a synonym of P. mexicanus by Miyazaki himself, the discoverer of P. peruvianus. Furthermore, P. ecuadoriensis is suggestive of a synonym of P. mexicanus by Miyazaki. I also regard P. amazonicus, P. inca and P. caliensis as dependent species. I entertain a strong suspicion that these are synonymous with P. mexicanus. Therefore, P. mexicanus is the only species that distinctly inhabits Latin America. In the classification, in particular Paragonimus, we have to recognize the variations in the same species. |

| Abstract | 本研究の目的は, 1)県下中核病院の地域医療連携,患者支援活動の現状について面接調査を行い,その実態を明らかにすること, 2)その結果をもとに,今後の特定機能病院に求められる地域医療連携や患者支援のあり方を検討する上での参考にすること,の2点にあった｡県下10か所の地域中核病院を対象に,地域医療連携業務やその他の患者支援活動に関する質問紙を作成し,面接調査を行った｡その結果,医療連携部門の歴史や業務内容は様々であったが,どの病院も地域医療連携部門を設置することで,開業医との連携を強化し,紹介･逆紹介の円滑化を図ろうとしている状況がうかがえた｡特に連携業務の中 でも,開業医からの紹介患者を病院にスムーズに受け入れる機能を重視している傾向が見られた｡また,すべての対象病院が地域医療連携以外の患者支援活動を行う部門を有しており,医療相談や地域の医療保健施設との連携強化等の業務を主に行っていた｡これらの調査結果をもとに,特定機能病院である大学病院に求められる地域医療連携･患者支援のあり方について考察した｡ |

| Abstract | 広島県福山市のとちく場で1頭の牛の盲腸より22匹の吸虫を検出し､形態的特徴より総て平腹双口吸虫Homalogaster paloniaeの成虫と同定した｡この牛は岡山県真庭郡川上村で生まれ､そこで約10ヶ月飼育されていた｡福山市の牧場内には､この吸虫の中間宿主であるヒラマキモドキガイが生息するような環境はなく､岡山県で飼育されていた時にこの吸虫に感染した可能性が高いと推察された｡ |

| Abstract | The aim of this study was to develop a portable device (IM checker) for measuring dental implant mobility using a microcontroller. A constant amplitude and frequency vibration was applied to a dental implant model and the acceleration signal was detected using measuring probe. Then the IM score was obtained using the criteria developed in this study. We made several implant models of different implant lengths, diameters and material of Rigolac® or Molteno®. There was a linear relationship between mechanical mobility at 400 Hz of the models and the IM scores (R(2) = 0.92). The IM checker could discriminate the mobility of dental implant models in twelve measurements with P < 0.01. There was no significant difference in the means of the IM score measured by four operators with P < 0.01. The results indicated that the IM checker had sufficient reliability and could be available in dental clinics. |

| Abstract | Thermotolerance in tsAF8 cells develops during incubation at 34℃ after heating at 45℃, while it is suppressed by the following incubation at a non-permissive temperature of 39.7℃ after the same heating. The incubation temperature after heating may affect the cell cycle and consequently thermotolerance. In the present study, a relationship between the thermotolerance and the cell cycle of tsAF8 was investigated. The cell cycle fractions and DNA synthesis were measured by flow cytometry using double staining with propidium iodide and bromodeoxyuridine. When the tsAF8 cells were heated at 45℃ for 20 min, and thereafter incubated at 34℃, bromodeoxyuridine uptake in the S phase cells (DNA synthesis) was recovered to 65.1% 6 h after the heating, and the cells showed gradual accumulation in the G(2)/M phase. When the cells were incubated at 39.7℃ after heating at 45℃ for 20 min, then showed inhibition of thermotolerance development, the DNA synthesis was recovered to 15.1% temporarily 6 h after the heating, but it became 0% after 12 h, and the cells did not remarkably accumulate in any phases of the cell cycle. This inhibition of DNA synthesis at 39.7℃ was considered to be the result of cell survival decreasing by a step-down heating. However, the relationship between the thermotolerance and the cell cycle was not found out in tsAF8 cells, because the cells did not accumulate in any phases of the cell cycle under the inhibitory condition of thermotolerance. |
