Abstract Approximately 10 years have passed since video capsule endoscopy (VCE)was introduced in routine clinical practice in Japan. We started using VCE in January 2009. Here we investigated the contribution of VCE and its level of recognition at our 855-bed hospital. We compared the clinical characteristics of intra-hospital consultation cases with non-consultation cases, and the details of the cases from each of our hospital’s departments. The number of intra-hospital consultations was notably increased by our expansion of the indications for non-obscure gastrointestinal bleeding (OGIB)cases. However, most of our hospital’s departments are sub-departments of internal medicine, in which almost no change in indications has been made. We found that the percentage of OGIB cases was higher among the intra-hospital consultations than non-consultations, and that the OGIB percentages differed markedly among the departments. Especially for OGIB cases, all hospital departments should be better informed regarding the indications for and efficacy of VCE.

Abstract To perform computed tomography (CT)/magnetic resonance imaging (MRI) scans safely and reliably, adherence to a pre-scan checklist is crucial. However, physicians and nurses may differ in their recognition of the usefulness of this checklist. We conducted the present study to investigate possible differences between physicians’ and nurses’ awareness of CT/MRI scan safety in terms of their adherence to the pre-scan checklist. We prepared an independently developed self-administered 23-item questionnaire about attributes of subjects, CT/ MRI scan safety recognition and current situation for CT/MRI examinations. The survey was distributed to 468 participants who combined nurses and physicians at Okayama University Hospitalin Okayama, Japan. We analysis the responses of the 224 participants (117 nurses and 107 physicians)who returned the survey with on missing data (a 65.1% completion rate). The overall safety recognition scores were significantly higher for the nurses than for the physicians (p<0.001). In addition, the physicians did not sufficiently know or implement the guidelines in the CT/MRI scan safety manual used at our hospital. Nurses and physicians demonstrated marked differences in their awareness and knowledge of safety regarding CT/MRI scans. Measures for improving safety recognition should thus be designed for individual healthcare occupations.

Abstract The recent application of artificial intelligence(AI)to clinical medicine has confirmed the usefulness of AI for diagnostic imaging, histopathological examinations, and dermatologic screening. Clinical decision support systems are another promising area to which AI could contribute toward better clinical decisions. We have developed computer-assisted diagnostic support systems to reduce human diagnostic errors such as delayed diagnoses, misdiagnoses, and overdiagnoses. Our three Diagnosis Reminder(DR)systems include two AI systems that use machine learning in their diagnosis algorithms. Here, we compared the diagnostic accuracy of a DR-supported group with that of an unassisted physicians group, using three difficult patient cases provided by experts in general medicine.  Our analyses revealed that the three AI diagnostic systems could not provide accurate differential diagnoses up to top 10 in all three patient cases because of incomplete data inputs for machine learning. However, the first DR system, which was developed by an experienced diagnostician over the last 35 years, showed very useful performance in reducing human diagnostic errors when it was used by an expert physician. The use of AI diagnostic systems by knowledgeable physicians will lead to better diagnostic performance. We also discuss the current scenario, future challenges, and prospects for AI diagnostic systems herein.

Abstract A 78-year-old Japanese woman was referred to our hospital because of fecal occult blood. Barium enema and CT colonography revealed the herniation of the transverse colon and the greater omentum into the right hemithorax through a retrosternal defect. We diagnosed a Morgagni hernia, and we performed a laparoscopic hernia repair with a three-port approach. The invaginated transverse colon and omentum were easily reduced into the abdominal cavity. The size of the hernial orifice was estimated to be approx. 2.5×4 cm. We placed Ventralight ST® mesh over the hernial orifice without hernial sac resection, and we used the double-crown technique to fix the mesh with a hernia stapler. The postoperative course was uneventful, and the patient was discharged on the 5th postoperative day with no complications. There has been no recurrence at 2 years after the surgery. Laparoscopic repair is a safe, less-invasive and useful method for repairing a Morgagni hernia.
