Abstract We report a case of mandibular osteosarcoma in a Japanese woman in her 70s who was p16-positive. Despite the rapid growth of the tumor, the patient responded well to chemotherapy and was then able to undergo surgery. Head and neck osteosarcoma (HNOS) is a very rare cancer, and although the importance of surgery has been pointed out, the effectiveness of chemotherapy is unclear. Resection margin negativity and response to chemotherapy have been reported as prognostic factors; another report assessed the effectiveness of the immunohistochemical expression of p16 protein as a predictor of response to chemotherapy.

Abstract With rapidly aging populations, family care functions can become weakened, and community health services often lack unified standards. A standardized and professional community home-based long-term care model (CHLCM) for the elderly is urgently needed in many regions of China and in other countries. Here, we explored the indicators of the need for a CHLCM among elderly individuals, and we constructed a CHLCM. We created and distributed a questionnaire regarding the requirement of long-term care services, based on a literature review. The two-rounds Delphi method was used, involving 20 experts who were randomly selected from among the medical universities, community health service centers, and nursing homes in Nanning, Guangxi, China. The experts’ enthusiasm rates in the questionnaire’s two rounds were 95% and 100%, respectively. The authentic coefficient of the experts’ consulting was 0.857, and that of the experts’ academic level was 0.835; the judgement coefficient was 0.880 and the familiar coefficient was 0.855. The CHLCM includes service content and an evaluation. The coordination coefficients for the two primary, eight secondary, and 29 tertiary indicators were 0.200, 0.386, and 0.184, respectively (p<0.05). The experts’ enthusiasm and authority were high. The coordination of the experts’ agreement was sufficient, and the analysis results were reliable. The CHLCM includes 29 items that provide a foundation and references for the formulation of concrete indicators and subsequent research.

Abstract Bone marrow edema (BME) after meniscus injury and risk factors for subchondral insufficiency fracture of the knee (SIFK) have been reported. However, their association with medial meniscus posterior root tear (MMPRT) remains unknown. We investigated the association of BME volume (BME-V), posterior shinycorner lesion (PSCL), and SIFK with MMPRT to examine the correlations between BME-V and medial meniscus extrusion (MME), PSCL and duration from injury to the time of magnetic resonance imaging (duration), and SIFK and duration. Twenty-nine patients who underwent surgery for MMPRT were included (mean age, 59.2; range, 39-84). The presence of PSCL, femoral BME-V (cm3), and SIFK grade (1-4) were evaluated. Preoperative factors, such as MME (mm) and duration (weeks), were investigated using multivariate linear/ logistic regression analyses. Multivariate linear regression analysis revealed duration as a significant factor for high-grade SIFK (p<0.01). Multivariate logistic regression analysis revealed duration as a significant factor for the presence of PSCL (odds ratio=0.94, p<0.05). A long duration of MMPRT leads to severe MME and highgrade SIFK (3 and 4), often resulting in knee arthroplasty. Early diagnosis of MMPRT and pullout repair can prevent severe MME and high-grade SIFK.

Abstract 本研究は,社会参加学習の原理を小学校社会科に取り入れ,主権者育成を目指した教育内容開発を行うものである。具体的には,先行研究で明らかにされている社会参加学習の原理と方法を検討したうえで,小学校社会科において社会参加を学習指導に取り入れる方法を解明し,その方法を子供自身が地域課題の解決に取り組む単元の開発を通して提案する。主権者教育は,一般的には中等教育段階を想定したものと捉えられがちであるが,主権者育成はたんに投票を促すだけではなく,社会の形成者として必要な資質・能力を広く身に着けさせることを目指したものである。その資質・能力には,認識から思考力・判断力,そして,態度や行動まで含まれる。これを総合的に身に付けさせるためには,社会の中で自ら課題を発見し,その解決に取り組む学習が有効であると考えられる。本研究では,地域課題の解決を目指した社会科の単元開発を通して,初等教育段階の主権者育成の方法を示す。

Title Alternative The Transition of Inter-Prefectural Migration of University Entrants in Kyusyu Region

Abstract 本研究では,九州地方における大学進学移動の時系列的推移を分析した。その結果,1980年代以降,県別に程度の差はあるものの,自県内進学が増加していることが判明した。また,九州地方各県間の進学移動は全体としては増加傾向にあるが,県ごとに傾向が異なること,広域的には沖縄県を除いては南関東への進学移動が大幅に減少していることも明らかとなった。さらに,九州地方では,福岡県と熊本県が大学進学者を吸引する中心となっているが,福岡県の吸引力が圧倒的に大きく,1980 年以降,この傾向は大きく変わっていないことも確認された。これらの結果には,大学新設等による各県の入学定員増にともなう収容率の増加が影響していると考えられるが,佐賀,長崎,大分,宮崎の4県では,収容率の増加に伴い残留率と占有率がともに増加し,収容率の増加が自県内進学に寄与していること,福岡県と熊本県では,収容率は減少したものの残留率と占有率がともに増加し自県内進学が維持されていること,鹿児島と沖縄県では,収容率が減少し他県への進学が不可避となっていることが想定されることが判明した。

Title Alternative Big data in the sex chromosomes of Silene plants and a fungus acting as a plant sex chromosome

Abstract Silene latifolia (Caryophyllaceae) is a dioecious plant that has long been used for study on sex chromosomes in plants. The advantage but also disadvantage of S. latifolia as a model system is the size of the Y chromosome that contains an extremely large male-specific region (approx. > 500 Mb). This feature implies that the evolutionary history of sex chromo-somes remains in the S. latifolia Y chromosome, while the size makes analyses esoteric. Another advantage is that plants in the genus Silene show variation in reproductive systems; most are gynodioecy (females and hermaphrodites), which is thought of as an evolutionary status before establishment of dioecy (males and females), with a few hermaphrodites and dioecy, suggesting that the genus Silene may represent an epitome of the sex chromosome evolution. Microbotryum is a biotrophic fungi, whose infection causes masculinization of the female flower, as if the fungus acts as the Y chromo-some. Though the underlying molecular mechanisms remain unknown, recent high-throughput sequence technologies provide many candidate genes for sex determination in plants and sex conversion by the fungus. In this article, I review and introduce studies of the Y chromosome in S. latifolia plant, the evolution of sex chromosomes in the genus Silene, the masculinization of female flowers caused by a fungus infection, and a virus vector that can be used for genetic analysis of the key genes involved in these processes.

Abstract Both the ancient Maya and Kofun society of Japan were cultural regions formed by Homo sapiens groups after leaving the Eurasian continent more than 10,000 years ago. In this paper, we examine their similarities and differences by comparing perspectives on time and space. The two societies were stratified and witnessed the development of monuments that emphasized their verticality. The greatest similarity was the formation of a network society in which local polities coexisted without the institutionalization of centralized leadership at the beginning. Subsequently, the Kofun society adopted the worldview of the Eurasian continent including the concept of the "nation", thereby achieving a new social integration. On the other hand, the ancient Maya, a primary civilization, did not go through the same social process and maintained a sustainable society over a long period of time, maintaining the same form of worldview, monuments, and technological systems. On the similarities rooted in the universality of the human body and cognition, disparity in the process of environmental and social changes, including the direction and distance of "Out of Eurasia", can be seen in the subsequent differences between the ancient Maya and the Japanese archipelago. In this paper, we have illustrated how the similarities and differences created diverse directions of culture and history.

Abstract This study examined the relationship between skin physiological indices and pressure ulcers in elderly people. The subjects were 55 bedridden elderly Japanese patients with a median age of 85 years. The following parame-ters were measured using non-invasive devices: skin surface temperature, moisture content in the stratum corneum, moisture content in the dermis, transepidermal water loss as an index of skin barrier function, skin erythema and skin elasticity. The sacral and 2 heel areas were observed as sites predisposed to pressure ulcers. Within one month after measuring the skin physiological indices, we confirmed pressure ulcers of National Pressure Ulcer Advisory Panel classification Stage II or worse based on medical records. Among the 55 patients, 4 (7.3%) prospectively developed a total of 5 pressure ulcers within 16 days. Only the skin erythema score was significantly higher with than without pressure ulcers (p < 0.001). We performed a binary logistic regression analysis and confirmed a significant relationship between pressure-ulcer development and the level of erythema (odds ratio = 1.026; 95% confidence interval: 1.011-1.042). Skin erythema increased before the development of pressure ulcers. Taken together, our results show that the high skin erythema score can be a predictive indicator of pressure ulcers.
