Abstract Struma ovarii is a rare neoplasm that accounts for approximately 0.3オ of ovarian tumors. Due to its ultrasound morphology, which is quite similar to that of malignant ovarian carcinoma, most struma ovarii cases are open operated with laparotomy rather than laparoscopy. We present 3 cases of struma ovarii, which were diagnosed preoperatively by imaging studies and removed by laparoscopic surgery. All patients were premenopausal women between ages 31‒50. The magnetic resonance imaging (MRI) findings were complex masses composed of multiple cysts and solid components with T2-hypointense regions as well as multiple T1-hyperintense cystic areas, findings that are typical for struma ovarii. A combination of plain computed tomography (CT), positron emission tomography (PET)-CT, and scintigraphy was useful for diagnosis. Laboratory examination revealed elevated serum thyroglobulin, which led to the diagnosis of struma ovarii. Laparoscopic surgeries were performed without rupturing the tumors. Although it has been difficult to differentiate between struma ovarii and malignant tumors by conventional methods, recently MRI techniques appear make it possible to diagnose struma ovarii preoperatively from the abovementioned imaging characteristic, together with laboratory data. As for treatment, we think laparoscopy could be successful for struma ovarii, but the surgeon must be careful not to rupture the tumor intra-abdominally in order to prevent dissemination, which could lead to malignancy.

Abstract Atrophy of the gastric mucosa is a precursor of intestinal-type gastric cancer, and Helicobacter pylori infection causes atrophic gastritis. The aim of this study was to determine whether the genetic diversity of H. pylori virulence genes is associated with the development and progression of gastric atrophy in humans. We isolated and cultured H. pylori strains from patients with gastric ulcer and duodenal ulcer accompanied by atrophic gastritis in background mucosa. H. pylori strains were stored at -80℃ prior to the experiments being carried out. We analyzed iceA, babA, vacA, cagA, and cagE genes by PCR. The cagA gene was analyzed through sequencing of the C-terminal region containing the EPIYA motif, which is related to tyrosine phosphorylation. Severe atrophy was observed in patients with gastric ulcer. The major phenotype of the vacA gene was s1c/m1 (93オ). The cagA gene was detected in all strains. The cagE gene was not detected in 2 and 5 strains from the mild cases and severe cases, respectively. The major cagA EPIYA motif, which is amino acids repeat in the C terminus, was the A-B-D type (44 of 58 strains). The virulence genes were not statistically associated with the severity of atrophy in the background gastric mucosa in humans. Not only identification of bacterial virulence factors but also studies of the host response will be necessary to investigate the progression of gastric atrophy and subsequent cancer development in humans.

Title Alternative Regional Industrial Agglomeration and Home Market Effects: The comparison among three different specifications

Abstract The region with a large market tends to increase the number of firms. We can often see the phenomenon of expanding economic disparities between regions. Krugman described this phenomenon using theoretical model and also he defined the concept of home market effect. The home market effect means that the production activity is done over the ratio of market (demand volume) in the region itself. However, by changing the assumption such as Davis (1998), the home market effect does not exist in the theoretical model. Although various empirical studies have been tested, the home market effect has not been necessarily confirmed. In this paper, we analyze the home market effect using the three specifications. Two specifications out of the three specifications are derived from the new economic geography, the rest specification out of the three specifications is derived from international trade. In doing so, it is possible to test the home market effect from various angles, and consider the validity of home market effect. Looking at the overall empirical results, the home market effect is confirmed in a number of manufacturing industries. Also, the home market effect is always confirmed in the six heavy and chemical industries such as chemical industry.

Title Alternative Political-Economic Situation and the Financial Condition of Local Governments in Poland

Abstract This paper conducts a critical analysis on the political-economic situation of Poland and the current financial condition of the Polish local governments. Poland was the only country in the EU that avoided recession in 2009, i.e. just after the financial crisis it the world in 2008. While a number of studies suggest that the strong exports and individual consumption expenditures have been supporting the economy, they tend to miss the crucial fact that local government finance in the country is relatively well-balanced, and it became one of the factors of the economic stability. Aiming at examining such curious steadiness of the Polish local governments, this paper first focuses on the political and economic situation in Poland to understand the background of the financial condition of the local governments. Secondly, the system of local autonomy and its changes will be reviewed. Thirdly, the paper outlines the current political situation of the country, particularly the outcome of the general election held in 2012. Different regional strategies of the major political parties will be analyzed as well. Finally, the condition of local government finance will be examined to clarify their problems and challenges. From the analysis of this paper, the following points will be elucidated: (1) The solid economic performance has been maintained in Poland even after the global economic crisis. (2) The disparity of economic growth by prefecture and the disparity of the domestic product per capita by prefecture both show observable trend of its increase. (3) There is a big difference in the regional policy between the two major political parties in the country. The ruling party, the Civic Platform (Platforma Obywatelska; PO), bases its policy on the EU cohesion fund and adopts the economic growth model, in which the economies of metropolises are the engine to lead the country’s economy. On the other hand, the main opposition party, Law and Justice (Prawo i Sprawiedliwość; PiS), sets the liquidation of domestic disparities based on the “Polish” policies as its main goal. (4) The local government system of Poland is characterized by the great authority of commune (gmina) and the securing of tax revenue by the state. This model has been ensuring financial stability of the local government. (5) Many state-owned real estates have been changed to come under local government ownership during the period of systemic transformation. The incomes from the real estates and gains upon their sale have been contributing to the stability of local government finance. (6) Local governments have been one of the factors of Poland’s stable growth. However, many local governments recently started to fall into deficit. The causes of the deficit are as follows: a slowdown in tax revenues due to the recession; the falling real estate prices, the rising prices of construction materials for public works; the rising energy prices; and natural disasters that occur frequently in recent years. (7) The reduction policy of the state budget, directly bound up with the tighter condition of local government finance, as well as the decrease of financial supports from the EU Cohesion Fund and its related cuts in budgets for public works, has a negative impact on the local economy. They consequently affect the growth of the Polish economy to cause its slowing down.

Abstract Plain abdominal radiography is a very basic examination and plays an important role in primary care. The objectives of this study were to clarify colon distributions on plain abdominal radiographs. Forty-three healthy volunteers underwent gastric fluoroscopy, and 2 hours later, plain abdominal radiography in the supine position. A region of interest (ROI) was defined uniformly on each X-ray image to divide the image into 600 zones. The area corresponding to the large bowel within the ROI was divided into 4 segments (ascending colon, transverse colon, descending colon, and sigmoid colon+rectum). The percentage of barium in each segment relative to the total volume of barium used was calculated to evaluate the percent ROI occupancy. The large bowel covered 76.7% of the entire ROI, with the percent duplication being 55%. The duplicated area corresponded to the transverse colon region. When the method proposed by Arhan et al. was used, the percentage of the colon actually present in each segment relative to that determined theoretically was 99.6% for the right colon segment, 92.2% for the left colon segment, and 92.2% for the sigmoid/rectal segment. However, in cases in which the transverse colon descended partially from the fifth lumbar vertebra, the percentage occupied by the sigmoid colon+rectum decreased to 57.2%. We applied a new large bowel segmentation method especially for patients with ptosis, by devising a line joining the lateral side of the right lesser pelvis and the lower ends of both sacroiliac joints.

Abstract The objective of early rehabilitation after anterior cruciate ligament (ACL) reconstruction is to increase the muscle strength of the lower extremities. Closed kinetic chain (CKC) exercise induces co-contraction of the agonist and antagonist muscles. The purpose of this study was to compare the postoperative muscle strength/mass of subjects who performed our new CKC exercise (new rehabilitation group:group N) from week 4, and subjects who received traditional rehabilitation alone (traditional rehabilitation group:group T). The subjects stood on the device and maintained balance. Then, low-frequency stimulation waves were applied to 2 points each in the anterior and posterior region of the injured thigh 3 times a week for 3 months. Measurement of muscle strength was performed 4 times (before the start, and then once a month). Muscle mass was evaluated in CT images of the extensor and flexor muscles of 10 knees (10 subjects) in each group. The injured legs of group N showed significant improvement after one month compared to group T. The cross-sectional area of the extensor muscles of the injured legs tended to a show a greater increase at 3 months in group N. This rehabilitation method makes it possible to contract fast-twitch muscles, which may be a useful for improving extensor muscle strength after ACL reconstruction.

Abstract Recently, competition among universities has a whole new face, and emphasis is given toward interdependence and interactive evolution between universities and regions. From this social perspective, universities should contribute to regional development by serving the regional society using results of intellectual work, and it is necessary for universities to accomplish social contribution and regional cooperation strategically. One of the most important parts in strategic planning is that students participate in activities for social contribution and regional cooperation. This paper presents a survey of the university students’ awareness on social contribution and regional cooperation using several statistical methods and attempts to find factors which affect activities for social contribution and regional cooperation using logistic regression analysis.
