Abstract We prospectively assessed sexual function before and after a tension-free vaginal mesh ( TVM) procedure for pelvic organ prolapse ( POP) . From April 2007 to March 2009, a total of 42 patients, who reported having an active sex life preoperatively underwent TVM for POP. Female sexual function was evaluated with the self-reporting, multiple-domain Female Sexual Function Index (FSFI) . We administered the FSFI to all of the patients before and at 3, 6, and 12 months after surgery. The mean age at surgery was 61.3±7.1 years. Before the TVM surgery, the FSFI score was very low ( 12.5±9.0) . The total FSFI score was improved significantly at 12 months after surgery ( 17.4±7.7) . The Arousal, Lubrication and Orgasm domains were significantly improved at 12 months after surgery. POP appears to have a significant negative impact on female sexual function. Our findings indicate that TVM for POP improved sexual function in a group of Japanese women with POP.

Abstract 　ドロシー・オズボーンは，結婚前に，のちに外交官・著述家として名を成すウィリアム・テンプルに多くの手紙を書いている。本研究は，これまでまとまった形ではほとんど論じられてこなかったドロシーの手紙を取り上げ，それらを通して結婚までの経緯をたどりながら，彼女の人物像，手紙に含まれる文学的意義，テンプルの思想や生き方に与えたであろう影響等を探ろうとするものである。小論では，「ドロシー・オズボーンの手紙（Ⅰ）・（Ⅱ）」（本誌第161 号・162 号, 2016 年）の続稿として，秘密裏の婚約，ドロシーの天然痘罹患，さまざまな障害を乗り越えての結婚，ムーア・パークでのハネムーン等を伝記的に考察した。

Abstract 　児童生徒の運動に関連した現代的健康課題として，運動不足に伴う肥満などの生活習慣病と運動過多に伴う四肢および脊柱のスポーツ傷害が指摘されている。「運動器の10 年」日本委員会1）は，運動器疾患の罹患率を，６～７％と報告しており，「過度な運動，スポーツによる運動器疾患・障害を抱える子どももみられる状況」と指摘している。その流れを受けて，学校保健安全法施行規則の一部改正により検診項目に「運動器」が加えられ，平成28 年度より幼稚園から高等学校までの学校において運動器検診が義務化された。この検診を通じて，運動器疾患の早期発見やスポーツ障害を予防することが目指されている。しかし，平成23年の文部科学省の調査によると内科検診で運動器の検診している学校は，3.2 ～ 4.7％であり多くの学校で運動器検診は，これまで実施されていない。本研究は，平成27 年度に，義務実施に先行して小学校において運動器検診を行い，実施上の問題点や児童の運動器の現状を明らかにした。

Abstract 　本論は，エンゲストロームの拡張的学習の理論の意義を指摘し，その理論的基盤を解明することを目的とする。文化・歴史的活動理論に基づく拡張的学習の理論は，状況的学習論が十分に提示できずにいた，状況あるいは文脈を越境し，新しく共同体を再組織化するあり様について理論的説明を与えている。垂直的次元と水平的次元との弁証法として位置づけられる再組織化の過程は，言語を基盤としている。言語は，活動システムの媒介物として状況・文脈依存性を持ちながらも，同時に，それらの横断可能性を想定させる機能を有しており，それに拡張的学習は支えられている。

Abstract Childhood anaplastic large cell lymphoma (ALCL) accounts for approx. 10–30 of cases of non-Hodgkin lymphoma, and the ALCL99 study reported 60–75 disease-free survival; however, a relatively high relapse rate was observed (25–30 ). We report 2 patients with Stage III ALCL who relapsed 6–18 months after the end of ALCL99 chemotherapy. A retrospective molecular analysis identified the nucleophosmin (NPM)-anaplastic lymphoma kinase (ALK) fusion gene in the first diagnostic bone marrow samples taken from both patients. However, antibodies against the ALK protein appeared to be relatively low in the serum of both patients (×100 and ×750). An increase in chemotherapy intensity may be beneficial if Stage III ALCL patients are shown to be NPM-ALK chimera-positive in the first diagnostic bone marrow sample.
