| 抄録 | To clarify the preventive effects of 0.01% atropine eye drops against myopia progression and axial elongation in children, a meta-analysis was carried out based on data obtained from PubMed and Web of Science as of August 1, 2021. Randomized controlled trials (RCTs) that enrolled myopic children who had received atropine for at least one year were included in this study, Key search terms included myopia, children, and 0.01% or low-dose atropine. Heterogeneity was quantified by I2 statistics, and meta-analyses were performed using the fixed-effect model. Five RCTs involving 809 unique children were analyzed. One trial was excluded because of a poor Jadad score and markedly rapid myopia progression in controls. The mean effect sizes for 12 months in myopia progression and axial elongation synthesized from the remaining 4 RCTs were 0.20 (95% CI: 0.13 to 0.27) D and −0.08 (−0.11 to −0.04) mm, respectively (p<0.0001). The corresponding inhibition ratios were 28% and 19%. I2 statistics were 6% or less. Sensitivity analysis and funnel plots demonstrated the robustness of the estimation. The 0.01% atropine-induced inhibition ratio for myopia progression in Asian children was roughly half of that originally reported and did not reach the minimum requirement for clinical treatment. |

| 抄録 | In recent years, medical staff including physicians and nurses have been participating in home health care, reflecting the needs of an aging society in Japan. Pharmacists are also asked to work on home health care teams to ensure the medical safety of patients. It currently remains unclear whether direct communication, i.e. a meeting, between home-visiting physicians and pharmacists contributes to the proper use of medications and continuous medical care. We retrospectively analyzed the medication management guidance records of home-visited patients who received their first home visit between April 2014 and March 2017. We collected data on pharmacist inquiries, the duration of visits, and details from a meeting between home-visiting physicians and pharmacists. Thirty-five patients were included. At the first visit, the inquiry rate by pharmacists was 65.7%. The prescription question rate was significantly lower in patients with a meeting than in those without (p=0.033). The average duration of visits was significantly shorter for home-visited patients whose health care providers had a meeting (p=0.007). These results suggest that pharmacists who held a meeting with the home-visiting physician before the first patient visit were able to resolve drug-related issues earlier, which increased the work efficiency of home-visiting pharmacists. |

| 抄録 | The purpose of this paper is to clarify the role and position of Poland in the European production network, where the automobile industry is undergoing spatial expansion and upgrading. In the analysis of the automobile industry, Central and Eastern Europe is positioned as a periphery of Western Europe, and has many common factors in terms of growth after the transition to a market economy system. On the other hand, there are signifi cant differences in the growth process of the automobile industry in the four Central European countries. In the case of Poland, the continuation of the production system under the socialism economic system, the transformation of the fi nished vehicle production situation due to the change in the relationship with FIAT, and the acquisition of a role as a production base for engines and components in the European region can be seen. In terms of the production network, Poland has been integrated into a wider network in Europe and connected to a multilayered supply-chain as multinational automakers have changed their strategies in Central and Eastern Europe. Poland has also been producing EVs and other next-generation automotive products and receiving investment in them in recent years, which has led to the entry of new companies, signifi cant changes in production networks, and new developments such as state-owned companies leading domestic EV projects. Trade has also been expanding in line with the expansion of production, indicating an export-oriented industrial structure. There has also been a deepening of supply relationships within Central and Eastern Europe, particularly in the area of parts, and it has become clear that there has been a remarkable expansion of exports of fi nished vehicles and parts to Central European countries. |

| 抄録 | With rapidly aging populations, family care functions can become weakened, and community health services often lack unified standards. A standardized and professional community home-based long-term care model (CHLCM) for the elderly is urgently needed in many regions of China and in other countries. Here, we explored the indicators of the need for a CHLCM among elderly individuals, and we constructed a CHLCM. We created and distributed a questionnaire regarding the requirement of long-term care services, based on a literature review. The two-rounds Delphi method was used, involving 20 experts who were randomly selected from among the medical universities, community health service centers, and nursing homes in Nanning, Guangxi, China. The experts’ enthusiasm rates in the questionnaire’s two rounds were 95% and 100%, respectively. The authentic coefficient of the experts’ consulting was 0.857, and that of the experts’ academic level was 0.835; the judgement coefficient was 0.880 and the familiar coefficient was 0.855. The CHLCM includes service content and an evaluation. The coordination coefficients for the two primary, eight secondary, and 29 tertiary indicators were 0.200, 0.386, and 0.184, respectively (p<0.05). The experts’ enthusiasm and authority were high. The coordination of the experts’ agreement was sufficient, and the analysis results were reliable. The CHLCM includes 29 items that provide a foundation and references for the formulation of concrete indicators and subsequent research. |

| 抄録 | This study aimed to investigate the link between prolonged sedentary bouts and health-related quality of life (QOL) in patients on chronic hemodialysis (CHD). A total of 84 outpatients on CHD, aged 71.6±11.8 years, were enrolled in this cross-sectional study. Parameters for prolonged sedentary bouts [i.e., ≧ 30 min (% and bout) and ≧ 60 min (% and bout)] were measured using a triaxial accelerometer. Health-related QOL (HRQOL) was evaluated by the Euro-QOL (EQ-5D). Clinical parameters were obtained from medical records. Relatively prolonged sedentary bouts (%) were 44.0±18.2 (≧ 30 min) and 29.8±16.5 (≧ 60 min) for total days. Prolonged sedentary bouts (bouts) were 6.2±2.7 (≧ 30 min) and 2.7±1.6 (≧ 60 min) for total days. EQ-5D scores were 0.728±0.220. All prolonged sedentary bout parameters were negatively correlated with EQ-5D scores, except for prolonged sedentary bouts (≧ 60 min) (min) and relatively prolonged sedentary bouts (%) on hemodialysis days. Multiple regression analysis showed that prolonged sedentary bout parameters were an important factor in EQ-5D scores even after adjusting for confounding factors for total and non-hemodialysis days. Our results suggested that prolonged sedentary bouts were closely associated with HRQOL in patients on CHD, especially on non-hemodialysis days. |

| 抄録 | R-CHOP therapy is generally performed every 3 weeks. We investigated the effects of extending the interval of R-CHOP therapy for > 1 week on the prognoses of patients with non-indolent non-Hodgkin’s B-cell lymphoma. Among the 338 patients with non-indolent non-Hodgkin’s B-cell lymphoma who received initial chemotherapy at our institution, we focused on 178 patients who received R-CHOP therapy and analyzed the outcomes of the patients stratified by the treatment intervals. The estimated 3-year overall survival (OS) for the entire population was 82.1%. Patients treated at intervals of ≥ 4 weeks were significantly older, and they had significantly longer follow-up periods and lower relative dose intensity. But the estimated 3-year OS was comparable to those treated at <4 weeks (83.3% vs. 80.5% p=0.947). In a multivariate analysis, age and the dose of anti-cancer agents had significant impacts on OS, but there was no significant relationship regarding the treatment intervals. Propensity score matching confirmed the same result. R-CHOP therapy every around 4 weeks could achieve relatively good survival in some selected patients with non-indolent non-Hodgkin’s B-cell lymphoma. |

| 抄録 | Optogenetics, a technology to manipulate biological phenomena thorough light, has attracted much attention in neuroscience. Recently, the Magnet System, a photo-inducible protein dimerization system which can control the intracellular behavior of various biomolecules with high accuracy using light was developed. Furthermore, photoactivation systems for controlling biological phenomena are being developed by combining this technique with genome-editing technology (CRISPR/Cas9 System) or DNA recombination technology (Cre-loxP system). Herein, we review the history of optogenetics and the latest Magnet System technology and introduce our recently developed photoactivatable Cre knock-in mice with temporal-, spatial-, and cell-specific accuracy. |

| 抄録 | L-Lysine α-oxidase (LysOX) from Trichoderma viride is a homodimeric flavoenzyme that catalyzes the oxidative deamination of L-Lysine to produce α-keto-ε-aminocaproate with ammonia and hydrogen per-oxide. LysOX inhibited the growth of cancer cells but showed relatively low toxicity for normal cells. The full-length cDNA consists of 2,119 bp, and encodes a long N-terminal propeptide composed of 77 resi-dues (Met1-Arg77) and the mature protein (Ala78-Ile617). The LysOX gene was heterologously expressed in Streptomyces lividans TK24 or Escherichia coli SoluBL21. The enzymatic properties of the purified recombinant LysOX, such as substrate specificity, kinetic parameters and thermal stability, are the same as those of the native LysOX. The LysOX precursor (prLysOX) expressed in E. coli shows weak enzymatic activity and is activated by proteolytic processing. The crystal structure of prLysOX revealed that the propeptide of prLysOX indirectly changes the active site structure to inhibit enzyme activity. Moreover, the crystal structures of LysOX and its L-Lysine complex revealed that the hydrogen bonding network formed by Asp212, Asp315 and Ala440 with two water molecules is responsible for the recogni-tion of the ε-amino group of L-Lysine. In addition, a narrow substrate-binding site and acidic surface at the active site entrance both contribute to strict substrate specificity. Mutational analysis demonstrated that Asp212 and Asp315 are essential for substrate recognition, and the D212A/D315A LysOX prefers aromatic amino acids. Furthermore, the structural basis of the substrate specificity change has also been revealed by the structural analysis of the D212A/D315A LysOX and its substrate complexes. |

| 抄録 | Influenza potentially has a high mortality rate when it affects the elderly. We aimed to examine the differences in clinical manifestations in patients with influenza according to their age. This multicenter prospective study was performed in six medical institutions in Okayama and Kagawa prefectures (Japan). Between December 1, 2019 and March 31, 2020, we collected data on adult patients diagnosed with influenza type A, who were strat-ified into younger (20-49 years), middle-aged (50-64 years), and older groups (≥ 65 years). We compared the presence or absence of fever, respiratory symptoms, and extrapulmonary symptoms according to age group. In total, 203 patients (113, younger; 51, middle-aged; and 39, older) were eligible for the analysis. The maxi-mum body temperature and temperature at first physician visit in the older group were significantly lower than those in the younger group. The incidence of respiratory symptoms was not different among the three groups. Chills, muscle pain, and arthralgia as systemic symptoms were noted significantly more frequently in the younger (80.9%) and middle-aged (75.5%) groups than in the older group (51.3%) (p = 0.002). Fever and sys-temic symptoms were less likely to appear in older patients, possibly resulting in the delaying of hospital visits among older adults. |
