kn-abstract=In high myopia, eye dislocation due to increased globe volume or tight orbital volume causes acquired esotro-pia. GOR (globe/orbit volume ratio), an indicator of the degree of progression of this pathology, was investi-gated the relationships among easily obtained clinical parameters. In this retrospective study, 20 eyes from 10 acquired esotropia patients with high myopia but without abduction limitations were examined. The mean age of the patients was 63.7 ± 8.2 years (mean ± standard deviation). Volumes were measured on the three-dimen-sional fast imaging employing steady-state acquisition magnetic resonance imaging images using the vol-ume-measurement function. Correlations between GOR and the displacement angle of the globe (DA), axial length (AL), and equatorial diameter (ED) were investigated. Mean DA, AL, ED, and GOR values were 107.5 ± 8.5°, 28.86 ± 1.92 mm, 25.00 ± 1.16 mm, and 0.36 ± 0.05, respectively. Only AL was correlated with GOR (p < 0.0001, R2 = 0.6649); DA (p = 0.30, R2 = 0.0633) and ED (p = 0.91, R2 = 0.0008) were not. AL was the only clinically available parameter to indicate globe/orbit volume imbalances in acquired esotropia with high myopia but without abduction limitation. AL may be important for the clinical assessment of the progression of this pathology.

kn-abstract=Purpose: The purpose of this study was to establish and analyze a cell model of Leber congenital amaurosis type 16 (LCA16), which is caused by mutations in the KCNJ13 gene encoding Kir7.1, an inward-rectifying potassium ion channel.
Methods: The two guide RNAs specific to the target sites in the KCNJ13 gene were designed and KCNJ13 knock-out (KO) human-induced pluripotent stem cells (hiPSCs) were generated using the CRISPR/Cas9 system. The KCNJ13-KO hiPSCs were differentiated into retinal pigment epithelial cells (hiPSC-RPEs). The KCNJ13-KO in hiPSC-RPEs was confirmed by immunostaining. Phagocytic activity of hiPSC-RPEs was assessed using the uptake of fluorescently labeled porcine photoreceptor outer segments (POSs). Phagocytosis-related genes in RPE cells were assessed by quantitative polymerase chain reaction.
Results: Most of the translated region of the KCNJ13 gene was deleted in the KCNJ13-KO hiPSCs by the CRISPR/Cas9 system, and this confirmed that the Kir7.1 protein was not present in RPE cells induced from the hiPSCs. Expression of RPE marker genes such as BEST1 and CRALBP was retained in the wild-type (WT) and in the KCNJ13-KO hiPSC-RPE cells. However, phagocytic activity and expression of phagocytosis-related genes in the KCNJ13-null hiPSC-RPE cells were significantly reduced compared to those of WT.
Conclusions: We succeeded in generating an RPE model of LCA16 using hiPSCs. We suggest that Kir7.1 is required for phagocytosis of POSs by RPE cells and that impaired phagocytosis in the absence of Kir7.1 would be involved in the retinal degeneration found in LCA16.

kn-title=Risk factors for excessive postoperative exo-drift after unilateral lateral rectus muscle recession and medial rectus muscle resection for intermittent exotropia

kn-abstract=Background: To detect significant factors associated with excessive postoperative exo-drift in young patients with intermittent exotropia who had undergone unilateral lateral rectus muscle recession and medial rectus muscle resection.
Methods: We retrospectively examined the records of 64 consecutive patients < 18 years old who underwent surgery between April 2004 and December 2011. We sought risk factors for excessive postoperative exo-drift among patients' demographic and clinical characteristics using univariate and multivariable linear regression analysis.
Results: Younger patients (P = 0.007), and those with larger preoperative exo-deviation at distance (P = 0.033), a lower incidence of peripheral fusion at distance (P = 0.021) or a greater postoperative initial eso-deviation (P = 0.001), were significantly more likely to have an excessive postoperative exo-drift (> 20 prism diopters). Univariate analysis revealed significant associations between excessive postoperative exo-drift and age at surgery (P = 0.004), preoperative exo-deviation at distance (P = 0.017) and postoperative initial eso-deviation at distance (P < 0.001). Multivariable linear regression analysis showed that postoperative initial eso-deviation at distance (P = 0.008) was significantly associated with postoperative exo-drift.
Conclusions: Postoperative exodrift in unilateral RR is predicted by the initial postoperative eso-deviation, which may offset the overcorrection. However, the exo-drift is greater in cases with a large preoperative exo-deviation and/or at a younger age, and should be followed carefully.

kn-title=Formulas to Estimate Appropriate Surgical Amounts of Unilateral Recession-Resection in Intermittent Exotropia with Distance-Near Disparity

kn-abstract=The purpose of this study was to derive new formulas to provide an optimal surgical procedure and optimal amount of recession-resection (RR) surgery in intermittent exotropia (IXT) with a disparity in angle of deviation depending on the fixation distance. The records of 117 consecutive patients with IXT who underwent RR surgery between March 2008 and December 2011 at Okayama University Hospital were retrospectively examined. Multivariable linear regression analysis was performed using the observed corrective angle of deviation at distance or near fixation as the dependent variable, and amounts of lateral rectus muscle (LR) recession (mm) and medial rectus muscle (MR) resection, and age at surgery (years) as independent variables. Two simultaneous formulas were derived: corrective angle of deviation at distance fixation (°)=1.8×recession (mm)+1.6× resection (mm)+0.15×age (years)?6.6, and corrective angle at near fixation (°)=1.5×recession (mm)+1.7× resection (mm)+0.18×age (years)?3.8. Comparisons of coefficient values of the formulas between distance and near fixation revealed that LR recession was more affected by the corrective angle in distance than near fixation. MR resection was more affected at near than distance fixation. We found that our new formulas estimated the appropriate amount of unilateral RR surgery.

This study aimed to investigate the characteristics of the extraocular muscles and the orbital connective tissue pulleys in Japanese patients with age-related distance esotropia (ARDE) and high myopia using magnetic resonance imaging (MRI).
Methods
This was a retrospective case-series study. High-resolution coronal MRI scans of 12 orbits were obtained in 6 patients with ARDE and high myopia (age range: 51?69 years). We analyzed the images to determine the positions of the rectus muscle pulleys relative to the center of the globe, the integrity of the lateral rectus-superior rectus muscle (LR-SR) band, and the LR angle (the angle between the major axis of the LR and the vertical plane).
Results
The distance esotropia ranged from 4 to 25?, and 3 cases exhibited vertical deviations. The mean (±standard deviation (SD)) axial length was 28.5 (± 1.6) mm. The mean positions of the medial rectus muscle pulley and LR pulley were 1.3 mm inferior and 1.4 mm inferior, respectively, to those seen in the normal control group in our previous study (P?=?0.002 and P?=?0.05, respectively). All 12 orbits had abnormal elongated LR-SR bands, and 8 orbits (67%) displayed ruptured LR-SR bands. The LR angle (mean±SD; 18.8°?±?8.5°) increased significantly with the inferior displacement of the LR pulley (R2?=?0.77, P?=?0.0002).
Conclusions
Inferior displacement of the LR pulley and abnormal LR-SR bands were seen in Japanese ARDE patients with high myopia, as was found in ARDE patients without high myopia. The LR angle might be useful for judging the degree of LR pulley displacement.

kn-title=Two-year Results of Intravitreal Ranibizumab Injections Using a Treat-and-extend Regimen for Macular Edema due to Branch Retinal Vein Occlusion

kn-abstract= We investigated the effectiveness of a treat-and-extend regimen (TAE) of intravitreal ranibizumab injections for macular edema (ME) due to branch retinal vein occlusion (BRVO). We retrospectively examined 2-year results of 32 eyes of 32 patients who underwent TAE to treat ME due to BRVO. The patients whose treatment interval extended to ? 12 weeks were switched to a pro re nata regimen (PRN). For the patients whose treatment interval was <12 weeks, TAE was continued. At 2 years, 10 eyes had required no additional injections after the initial treatment period [recurrence(?) group], whereas the other 22 eyes required additional treatment [recurrence(+) group]. Among the recurrence(+) patients, 11 eyes (34.4% of total) were eventually switched from TAE to PRN; the other 11 eyes (34.4%) continued TAE for 2 years. Visual acuity and central retinal thickness were significantly improved in both the recurrence(+) and (?) groups, and there was no significant betweengroup difference in visual acuity at 2 years. Univariate analyses revealed significant differences in visual acuity (p=0.004), age (p=0.014), and vessel occlusion site (p=0.018) between these groups. Our results suggest that TAE may be effective for BRVO patients with lower visual acuity, older age, and occlusion of a major vein.

kn-title=Twist Knot: A New Sliding Noose in Adjustable Suture Strabismus Surgery

kn-abstract= To permit noose movement without fraying the sutures following strabismus surgery, we designed a new sliding noose, the “twist knot” and investigated its advantages and disadvantages. We measured the tensile strength required to move the twist knot in a tightly tied state (134±19 gf) and in a loosened state (21±7 gf), and that required to move the conventional sliding noose in a tightly tied state (48±14 gf), and used the Kruskal-Wallis test to compare them. A significant difference was observed among the three tensile strengths (p<0.001). The twist knot technique allowed easy sliding without the multifilament braided suture becoming frayed and a knot to be firmly fixed without slipping. However, if the 2 strings of the pole sutures exit from the sclera at 2 widely separated positions, the sliding noose may become slack. Therefore, the distance between the pole sutures should be small. The simple twist knot technique was found to be an effective approach following adjustable surgery of strabismus.

en-title=Educational Effects of Various Team Childcare Styles in Preschool Education

en-abstract= The purpose of this study is to classify various styles of team childcare practicing in preschools. We adopted a questionnaire method to preschool teachers in a prefecture to research it empirically. The classification of team childcare styles was discussed based on the results of our questionnaire by co-researchers who has enough experiences of childcare, and we could examine educational effect, characteristics, and some problems of each team childcare style. As a result, it is clear that all teachers in the same preschool become members of team, and they are strongly conscious of “all teachers for all children.” Based on the premise mentioned, three categories of team childcare styles are found; 1) A team childcare style to focus on teachers’ arrangement, 2) A team childcare style to put emphasis on contents of childcare and life, and 3) A team childcare style after a childcare time on child understanding, childcare and so on.
kn-abstract= 本論は,幼稚園で実践されているティーム保育について,その多様な形態を分類することを目的に,1県内全域の幼稚園に対する質問紙調査を行い,実証的に明らかにしたものである。ティーム保育の形態の分類は,保育経験豊富な研究協力者によって,質問紙調査の結果を基にしたグループディスカッションで検討され,各形態に見る教育効果や特性,運営上の課題が精査された。その結果,園内の全教師が1つのティームとなり,全園児を全教師で保育するという意識を強く持っていることが明らかになった。そのことを前提に,ティーム保育の形態について,次に示す3つの分類が見出された:@「教師の配置に注目したティーム保育の形態」,A「保育内容や生活を重視したティーム保育の形態」,B「幼児理解や保育等に関する保育時間外のティーム保育の形態」。

kn-abstract= We examined the effects of caffeine treatment on ovalbumin- and histamine-induced rhinitis mouse models and on corticosterone and catecholamine levels in mouse plasma. Treatment with caffeine decreased sneezing and nasal rubbing in both models, but direct nasal administration had no effect. Plasma levels of corticosterone and catecholamine, which have known anti-allergy effects, were increased by caffeine treatment. The increases in corticosterone and catecholamine may be associated with the suppression of rhinitis symptoms.

en-title=An Attempt at Curriculum Improvement through an Evaluation by External Experienced Childcare Workers

en-abstract= We commissioned external seven childcare workers with experience of early childhood education and care to evaluate the curriculum of a nursery school. Evaluation results reflected wide points of view based on the experiences of each evaluator. It was possible to specify the items of "unification of transcription and expression, and adoption of the plain style," "making of a plan in conformity with originality in facilities and the reality of the child," "making of a preservation of health plan," "reorganization by the PDCA cycle," and "confirmation between the staff" as factors related to curriculum improvement. The validity of the external evaluation by the childcare workers with experience was confirmed as a policy of curriculum improvement.
kn-abstract= 本論は,保育現場における園のカリキュラム・マネジメントを活性化させるために,園外の保育経験者にカリキュラムの外部評価を依頼し,その評価内容と一連の評価方法について検討するものである。具体的には,地方政令市にあるA 保育園のカリキュラムを取り上げ,保育所保育士経験者及び幼稚園教諭経験者によって構成させる7名の外部評価者から,カリキュラムの評価を得た。その結果,外部評価者の勤務経験の違いを反映した,幅広い観点から,カリキュラムの評価できる点や問題点を確認することができた。カリキュラム改善の観点としては,「表記・表現の統一と分かりやすい様式の採用」「園の独自性や子どもの実態に即した計画の作成」「保健計画の作成」「PDCA サイクルによる再編成」「職員間での確認」の項目を明示することができた。カリキュラム改善の一方策として,保育経験者による外部評価に一定の有効性を確認した。

kn-title=Paecilomyces lilacinus-induced Scleritis Following Bleb-associated Endophthalmitis after Trabeculectomy

kn-abstract=Paecilomyces lilacinus (P. lilacinus) is a rare cause of fungal scleritis. We herein report a case of P. lilacinus-induced scleritis following bleb-associated endophthalmitis after trabeculectomy that was successfully treated with surgical excision of the affected sclera in combination with antifungal medication. An 85-year-old female underwent trabeculectomy of the left eye. A dellen formed in the corneal periphery due to limbal elevation of the filtering bleb and progressed to an infectious corneal ulcer, leading to blebitis. Eight days after the onset of blebitis, the patient was diagnosed with endophthalmitis, which resolved after vitrectomy. The growth of P. lilacinus was identified on swabs of the conjunctiva and the corneal specimen. Scleritis developed after the resolution of the endophthalmitis, and an early excision of the affected sclera, in addition to antifungal medication, resolved it completely. However, the scleritis recurred in a different region of the left eye. After 7 months of antifungal medication, the left eye showed no residual infection. When treating P. lilacinus-induced scleritis, surgical excision of the affected sclera has been shown to be an effective treatment strategy. Nevertheless, it is possible that the infection may recur in another part of the eyeball after the complete resolution of the primary lesion.

kn-abstract=リンク機構は,多種類の動きを生成することから,多くの動くおもちゃの基本メカニズムとして,欠かせない存在である。多節リンクを適用すれば,それぞれのリンクで各種動きを生成できるが,一方で,動きが一定にならなかったり,動かなくなったりする場合は多い。そこで本研究では,1つのリンクを駆動用に固定した5節リンク機構に注目し,目的の動きを得るための5節リンク機構の解法アルゴリズムの展開と,それを用いた設計方法について提案する。そして,適用例を示し,提案した設計方法の有用性について考察する。

kn-title=POEMS Syndrome in a 20-year-old Patient Diagnosed Following a Complaint of Reduced Visual Acuity

kn-abstract=We report a case of POEMS syndrome in a 20-year-old patient diagnosed after visiting an eye clinic with a chief complaint of reduced visual acuity. A male university student aged 20 years was referred to our department complaining of blurred vision in both eyes that had persisted for 1 month. He also noted headache, nausea, and paresthesia in the lower extremities around the same time. The visual acuity of his right and left eye was 20/40 and 20/20, respectively. Optic disc edema and serous retinal detachment were present. Brain magnetic resonance imaging showed no intracranial abnormalities, while elevated cerebrospinal fluid pressure, reduced nerve conduction velocity in both lower extremities, hepatosplenomegaly, M proteinemia, high blood VEGF levels, osteoblastic and osteolytic changes in the spine, and atypical plasma cells in bone lesions were noted. From the above findings, the patient was diagnosed with POEMS syndrome. He received high-dose dexamethasone, thalidomide, and radiotherapy on the sacral mass, followed by high-dose melphalan with autologous stem-cell support, and showed subsequent systemic and ophthalmologic improvement. Here, we report the youngest case ever of POEMS syndrome with ocular manifestation. If patients have optic disc edema in both eyes with no intracranial space-occupying lesion, POEMS syndrome should be considered in differential diagnosis, regardless of age.

kn-abstract=The author has had an experiment on the changes by death, of the vegetative Plankton (diatoms) in the organs of drowned bodies at winter, and has acquired the following results: 1) There were the 11 Diatoms were found in the water of the river used for this exepriment. 2) Most Diatoms were found in the lung and femur even two months after death but in the heart, liver and kidney in which each organs became softon, found the Diatoms only one month after death. Even after treating those organs, just a few Diatoms could be found. 3) Among 11 Diatoms, Cymbells parva, Melosira islandica and Cyclotella comta were considered in the strength for resisting to decomposition; and it was Tabellaria fenestrata which was not found at the earliest time.

kn-abstract=The author, continuing the experiment of spring and autumn, had an experiment on the changes by death, of the vegetative Plankton (diatoms) in the organs of drowned bodies in summar, and has obtained the following results: 1. There were 11 diatoms from the water of the river as well as the experiments of spring and autumn. 2. There were 8 among 11 kinds of diatoms which were found in the lung for three days after death, the utmost limited time for use. 3. There were 3 among 11 kinds of diatoms which were found in the heart, liver and kidney for three days after death, the utmost limited time for use, they are Cymbella parva, Melosira islandica and Cyclotella comta which have been considered to have strength for resisting to decomposition. 4. Among these 11, 5 diatoms such as Cymbella parva, Navicula radiosa, Melosira islandica, Cyclotella comta, and Rhopalodia giba were found in the femur even two months after death. 5. From the experiment above, the author has come to the conclusions. In summar when the progress of decomposition is quick, the organs of drowned bodies melt and it is for use, the diatoms stay in the femur for the long term; this can be said to be the basis in the explanation whether drowning is the cause of death.

Among these 11, 8 diatoms such as Cymbella naviculiformis, Cymbella parva, Navicula Placentula, Navicula radiosa, Nitzschia filiformis, Melosira islandica, Cyclotella comta and Rhopalodia gibba were found in the lung on the 20th day after death and in the femur on 60th day. 2) There were 2〜4 among 11 kinds of diatoms which were found in the heart, liver and kidney on the 20th day after death, the utmost limited time for use. 3) The diatoms which has been considered in the strength for resisting to decomposition were Cymbella parva, Melosira islandica, Cyclotella comta and Rhopalodia gibba. 4) From the results of these observations, the author has acquired the knowledges, which discovered the diatoms from the organs such as lung, heart, liver, kidney and femur of drowned bodies as far as they can be taken (used). This is supposed to be the basis in determining whether drowning is the cause of death.

kn-abstract=Nematol were administrated two young women of twenty-two for the treatment of their anchylostomiasis. It is a particular case, that a pharmacist misgave to the patients fifteen tablets as dosage at the same times. One women of them died of general weakness on the about twelve hours after the internal use. On the post-mortem examination, the most prominent findings are findings of oedema in the lungs and brain, fatty degeneration and cloudy swelling of the liver and the kidney, and bleeding of the submucous tissue of the stomach and the intestines.

kn-abstract=We investigated the relationship between the frequency of basal ganglia calcification appearing in CT scans and clinical symptoms in 3,599 patients. Basal ganglia were calcified in eleven patients (0.3%), of whom seven had calcification of the bilateral G. Pallidus, but no clinical symptoms. A relationship between calcification and clinical symptoms was shown in two patients under ten years old, a three-year-old with young brain atrophy and a six-year-old with a gonadotropin-producing brain tumor. Especially in patients over forty years old, a direct relationship between G.P. calcification and clinical symptoms was not shown, and calcification was considered to be pyhsilogical.

kn-abstract=Since Bast et. al. reported that patients with ovarian carcinoma had elevated levels of CA125 in serum, the attention has been paid to CA125 in serum as a tumor maker. Serum CA125 levels in 88 patients with gynecological disease were measured. Among 15 patients with malignant ovarian tumor, CA125 values were above 35 U/ml in 80% , and above 65 U/ml in 73% , of serum samples assayed. By increasing the values of CA125 considered normal from 35 to 65 U/ml, the positive rate of benign ovarian tumor and carcinoma of the uterus was markedly reduced. Therefore, it was more efficient as a screening test for gynecological disease that the value of CA125 considered abnormal be above 65 U/ml. Serum CA125 levels were correlated with efficacy of therapy and the clinical course.

kn-abstract=Percent depth dose curves of Lineac electron beam and Dermopan soft X-ray, which are used for radiation therapy of surface tumors, were studied in 2 patients with angioblastoma (Nakagawa) who had been followed up for a long time after radiation therapy. The results are discussed with a review of the literature. Lineac electron beam therapy at low doses that do not cause remote complications appeared to be useful in the treatment of surface tumors selected after careful consideration of the indications.

kn-abstract=Serum ferritin and serum carcinogenic embryonic antigen (CEA) levels in 153 patients (male: 112, female: 41) with primary lung cancer were simultaneously measured and compared with the histological type, clinical stage and clinical course. No correlation between serum ferritin and CEA levels was found. A higher percentage of positive test results for both serum ferritin and CEA was associated with advanced stages and metastasis in other organs. From the point of view of histological classification, high ferritin levels were mainly found in the patients with small-cell types of cancer. In contrast, high CEA levels were found in patients with adenocarcinoma. Therefore, the measurement of serum ferritin was not efficient as a screening test for lung cancer. Serum ferritin levels were, however, correlated with the efficacy of therapy and the clinical course.

kn-abstract=Magnetic resonance imaging (MRI) of pathological lymphnodes was reviewed in 12 patients with malignant lymphoma. We examined the characteristics of the images and T(1) values of lesions. Short SE was the most appropriate pulse sequence to delineate the lesions. It was intermediate in signal intensity between that of skeletal muscle and that of fat tissue. On IR image, the signal intensity of skeletal muscle or lung tissue was similar to that of lymphnodes, and on long SE image, that of fat tissue was similar to that of lymphnodes. It was difficult to differentiate Hodgkin's disease (HD) from non Hodgkin's lymphoma (NHL) using only T(1) values. The long SE signal intensity of the latter was higher than that of the former. The distribution of NHL was wider than that of HD. Though MRI has various problems such as long scan time and respiratory motion artifacts, it has various merits which other methods do hot have. We consider MRI to have an important role in the diagnosis and follow up of patients with malignant lymphoma.

kn-abstract=We analyzed calcifications in 557 computed tomography films of the head. Adnormal findings of calcification of vertebral arteroes were obrained in 36 of the 577 cases. In the 36 cases, vertigo was observed in 24 cases (66.7%), and brain atrophy in 10 cases (27.2%). The calcification of vertebral arteries (arteriosclerosis) correlated significantly with vertigo. Patients with vertigo should be examined by computed tomography for calcification of vertebral arteries at the level of the foramen magnum.

kn-abstract=In legal medicine, it is an established procedure to determine blood levels of ethanol in the living body or the corpse and estimate the level of drinking after establishment of the fact of drinking. In this process, effects of external factors, such as differences in the individual bodies, physical condition, various diseases, impact of food or drugs, exercise and bathing, present a problem. This paper describes a study on changes in blood levels of ethanol and its metabolites under the influence of varied factors as viewed from the practice of legal medicine. When rabbits were orally given Japanese "sake", there was a positive relationship between the dose of ethanol and its levels in the blood, whereas there was no such relationship with acetaldehyde or acetate. In the forced walk experiment in rabbits, no changes could be found in the levels of ethanol or acetate, but high levels of acetaldehyde were found in some animals. When the animals were bathed in hot water, acetaldehyde disappeared in the blood rather quickly in some rabbits, but the blood levels of acetate remained unchanged. Concurrent administration of sugar and Japanese "sake" to the experimental animals tended to acelerate metabolism both of acetaldehyde and acetate as well as ethanol. Administration of cyanamide elevated the acetaldehyde levels, and similar effect occurred with ethanol and acetaldehyde levels, in rabbit with a CCl(4)-damaged liver. In rabbits given ethanol every other day for 10 months, normal function was maintained in the liver, and high blood levels of ethanol were found in some animals. On the other hand, no significant changes occurred in acetaldehyde or acetate levels.

kn-abstract=Histochemical staining methods for alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH) were studied. Many reports on ADH staining have been documented, and in the current study the author attempted to stain cryostat-prepared tissue sections on the slide glass. The author studied the composition of the incubation medium, incorporating nitro-blue-tetrazolium (NBT) and the staining procedure. ALDH staining has been less documented, but the author obtained a satisfactory result by using a staining method basically the same as that for ADH, with disulfiram as the inhibitor. In vivo distributions of ADH activity were studied in man, rabbit, quinea pig, rat and mouse. The activity varied sagnificantly from species to species. It was the highest in the liver, and found in the mucous membrane of the digestive tract, lung, pancreas and kidney to a lesser extent. In addition, ALDH activity in man was striking, not only in the liver, but also in the lung and kidney. It was also found in the mucous membrane of the digestive tract. When changes in ADH activity were studied in rabbits orally fed with ethanol every other day for one year, no such findings could be confirmed.

kn-abstract=We found a case of left-sided inferior vena cava(ivc) by chance upon upper abdominal CT examination. Left-sided ivc rarely presents clinical symptoms, but it is necessary to distinguish ivc from abnormal masses such as retroperitoneal tumors or lymphnodal swelling. We discussed left-sided ivc and the literature about it.

kn-abstract=At Okayama University Medical School, the clinical application of magnetic resonance imaging (MRI) started in August 1984, and 16 patients with aortic aneurysm have been imaged by short spin echo (TR400msec, TE40msec) with a Toshiba MRT-15A (resistive type of magnet with a strength of 0.15T).MRI demonstrated the intimal flap and enabled the differentiatian between true and false lumens from the signal intensity. Thrombi were shown with high intensity and were differentiated from fast blood flow, but in some cases clots could not be distinguished from slow flow.MRI in which multiple planes are imaged directly without contrast medium, is a very useful imaging modality for the evaluation of aortic aneurysm.

kn-abstract=We have used an NMR-CT scanner clinically since August 1 1984. We report herein the operating pulse sepuences, and present 5 cases of 226 cases that we have examined. From our clinical experience during the past four months, we describe the advantages and disadvantages of NMR-CT. The advantages are that: 1) it is possible to recognize blood vessels without contrast materials and to evaluate the blood flow 2) there are neither bone nor air artifacts, and 3) it is possible to get the tomogram in any direction. The disadvantages are that: 1) the examination time is very long, 2) there are artifacts from ferromagnetic substances placed in the body, 3) when palse sequence is changed, images are different from each other, and 4) because of respiratory motion artifacts, the images of the chest and abdomen are inferior to those of other parts of the body.

kn-abstract=Serum ferritin levels were determined in 75 hematologic, 1205 malignant, and 538 benign diseases. In hematologic diseases the serum ferritin level was generally low in iron deficiency anemia, but high in hemochromatosis, aplastic anemia, and leukemia. In malignant diseases the serum ferritin concentration was increased remarkably in lung, liver, biliary, and urogential diseases and in malignant lymphoma. Among benign diseases hepatitis cases showed a tendency for high serum ferritin levels. Serum ferritin levels are influenced by various factors, and their careful evaluation may be essential for understanding of the underlining disease process.

kn-abstract=The serum and urinary ferritin levels and peripheral blood or synovial fluid leukocyte ferritin contents of 76 patients with rheumatoid arthritis (RA) were measured by the 2-site immunoradiometric assay method. Serum ferritin (s-fer.) levels of RA patients (mean±SD=33.9±26.3ng/ml, n=45) were within the normal range, although the s-fer. level correlated well with the amount of C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), γ-globulin level, RA test and disease activity. Urinary ferritin (u-fer.) levels of RA patients (8.7±12.7ng/ml, n=45) were not significantly different from controls (5.1±4.1ng/ml, n=23). However, when RA patients were divided into groups according to the therapy received at time of measurement, u-fer. level of RA patients under gold therapy (14.1±20.2ng/ml, n=15), but not those of patients under other treatments, were significantly higher than control level. The U/S ratio also tended to be higher. In addition, a remarkably high u-fer. level (over 300ng/ml) and U/S ratio (5.84) were found in are RA patient with gold nephropathy, indicating ferritin synthesis in kidney tissue. Ferritin contents of the supernatants of leukocytes were measured. Synovial fluid leukocytes (PMN : 29.7±9.5 fg/cell (n=16 ; MNC : 62.4±7.1 fg/cell (n=16)) tended to have higher ferritin contents than peripheral blood leukocytes (PMN : 5.3±2.6 fg/cell (n=16) ; MNC : 9.3±3.3 fg/cell (n=16)). There was no significant correlation between the ferritin contents of PMN or MNC of peripheral blood or synovial fluid and other parameters such as ferritin levels, ESR, CRP or RA test, except for a positive correlation between the ferritin contents of peripheral blood PMN and serum iron levels.

The cases are as follows : 1)two cases of incomplete cardiac rupture dur to thoracic trauma, 2)rupture of herniated large intestine due to abdominal trauma, 3)large incised wounds on the side of the neck by a small kitchen knife, 4)a homicidal case with similar wounds that accompanied self-inflicted wounds, 5)a homicidal case accompanied by multiple stab wounds on the chest, 6)an accidental death skewered by a slender metallic stick, 7)special fatal shotgun injuries, 8)a homicidal case accompanid by various burns by an electric hair-dryer, iron and ciga-rettes, 9)two murder cases by a metallic baseball bat, 10)a suicidal case due to a fall from a high place, and 11)a severely disrupted victim of a road traffic accident.

en-abstract=高度好熱性細菌 Thermus thermophilus HB8 由来アラニンラセマーゼ遺伝子を大腸菌中にクローニングし、発現させた後に、精製及び性質検討を行った。alr遺伝子は1080bpからなり360アミノ酸残基 HB8をコードしていたので、本酵素は38,596Daの分子量であると予想された。alr遺伝子の [ G + C ] 含量は、72%であり、Tm値は98.8℃であった。T.thermophilus HB8由来アラニンセマーゼを中等度好熱性細菌 Geobacillus stearothermophilus 及び赤痢菌 Shigella sonnei 由来アラニンラセマーゼと一次配列の比較をしたところ、G.stearothermophilus 由来のものと33%、赤痢菌由来の酵素を28%の相同性を示した。T.thermophilus HB8 由来アラニンラセマーゼを、70℃で10分間の熱処理後、DEAE-トーヨーパール陰イオン交換カラム等により精製した。精製酵素の最適温度は、d-アラニンからl-アラニンへの反応で55℃、l-アラニンからd-アラニンへの反応では60℃であり、最適pHは、9.0?10.0であった。また、70℃で30分インキュベーションを行った後にも、活性の低下は見受けられず耐熱性を示した。更に、本酵素は分子量38,000モノマー酵素であると推定され、その反応機構に興味が持たれる。
kn-abstract=An alanine racemase (EC 5.1.1.1) from an extreme thermophilic bacterium Thermus thermophilus HB8, was purified and characterized, and its gene was cloned. The cloned alanine racemase gene (alr) was expressed in Escherichia coli JM 109. The alr gene is composed of a 1080 bp and encoded a 360 amino
acid, and was predicted to have a molecular weight of 38,596. The enzyme was purified by heat shock at 70°C for 10min and DEAE Toyopearl 650M column chromatography. The purified enzyme had an optimum pH9.0?10.0 and an optimum temperature of 55°C?60°C. Enzyme activity was retained 100% after incubation of the enzyme at 70°C for 10min. Alanine racemase from Thermus thermophilus HB8 is a monomeric enzyme with a molecular mass of 39 kDa.

kn-abstract=Forty eyes in twenty-two cases in the active phase of retinopathy of prematurity were treated with laser photocagulation. Thiry (93.7%) of the 32 eyes in type I and intermediate type were found in the cicartrical phase grade 1. In type U in which cryocautery was used as combination therapy, only 2 eyes (25.0%) were found in grade 1, 2 eyes (25.0%) in grade 2, 1 eye (12.5%) in grade 4 and 3 eyes (37.5%) in grade 5. All 21 eyes in which photocoagulation was performed up to the middle of stage 3 were found in grade 1 (100%). However, among 11 eyes in which photocoagulation were performed in late stage 3, 9 eys (81.8%) were found in grade 1, and 2 eyes (18.2%) in grade 2. In conclusion, if adequate laser photocoagulation is performed in avascular ares at the middle of stage 3 of type T and intermediate type, treatment in cicartrical phase grade 1 is possible. Laser photocoagulation is clearly more effective than the xenon are photocoagula-tion or cryocautery. However, in type U it is difficult to control the condition by photocagulation alone, and early diagnosis followed by combination therapy using cryocautery is reqired.

kn-abstract=Hemodynamic and hormonal changes were studied in 13 sheep whose entire circulation was maintained by a biventricular bypass system with a pair of pusher-plate type blood pumps. Ventricular fibrillation was induced and pneumatically driven pumps placed outside the body kept the animals alive for 2-48 days (average: 11 days). Pump output and aortic pressure stayed within physiological ranges, but central venous pressure was elevated from 5 to 10-13 oHg after surgery. Serum levels of antidiuretic hormone, cortisol and insulin were stabilized soon after surgery. Adrenaline level and renin activity did not change significantly. For unknown reasons, the noradrenaline level showed a temporary increase after a week. Atrial natriuretic peptide (ANP) increased significantly 5 days after surgery. There were significant drops in triiodothyronine (T(3)) and thyroxine (T(4)) levels after surgery with gradual recovery afterward to 40% and 50% of the control levels, respectively. These results suggested that the biventricular bypass system maintained hemodynamic conditions similar to those maintained by the natural heart, although ANP, T(3) and T(4) values were changed.

kn-abstract=Thermal conductivity of soils in Shirakami Mountains that was registered on UNESCO's World Natural Heritage in 1993 were measured using a twin transient-state cylindrical-probe method. The typical results were as follows:(1) Thermal conductivity of the soils in Shirakami Mountains increased according to an increase of soil water content, which was the same tendency as the other soils. (2) The thermal conductivity of the soils was about 0.2〜0.3 Wm(-1)K(-1) smaller than volcanic ash soils, and it showed the same tendency as leaf mold.

en-abstract=ハイドロキサム酸のDIMBOAは特定のイネ科植物にあり、寄生する昆虫や病原菌に対して抵抗性物質となっている。一方、トウモロコシにはトウモロコシアブラムシとムギクビレアブラムシが寄生し、養分奪取、排泄する甘露によるスス病や萎縮病ウイルスの伝播者としての害をもたらしている。トウモロコシの21系統を1990年と1991年に栽培し、生育に伴うDIMBOA含量の消長とアブラムシの寄生の変化を調べた。DIMBOAは単位体重当り若齢期に多量を含有し、生育に伴って少量になり、老齢期になると検出不可能の量となった。使用した系統は全て多少ともアブラムシに対して抵抗性を示し、オオムギやコムギなどで見られた増殖は見られなかった。大増殖している農家の圃場から移植したが、DIMBOA含量の多い若齢期においては急速に死滅した。DIMBOA含量の少ない成熟期から老齢期にかけては若干管生存した。ムギミドリアブラムシを用いて、人工飼育液にDIMBOA等を添加して生存率を調べて、他の作用物質との関連を考察した。トウモロコシには同じ篩管に含有されるアコニット酸も多量にあり、両物質が共に抵抗性物質であると考えられた。
kn-abstract=2,4-Dihydrox-7-methoxy1,4-benzoxazin-3-one (DIMBOA), a hydroxamic acid is considered to be one of the components responsible for the resistance to pest insects in cereal plants. The relationship between concentration of DIMBOA and aphid infestation on 21 corn lines was investigated in 1990 and 1991. DIMBOA was detected in leaves od all corn lines tested, contained larger amounts in young plants and gradually decreased with growth. This property was the same as observed in wheat. A more than ten times difference in DIMBOA concentration was observed in corn lines. However, there was no positive correlation between DIMBOA concentration and aphid density. Most of the lines used had resistance to aphids. Resistance may be built together with other components such as (E)-aconitic acid.

en-abstract=コムギもオオムギと同様に系統・品種によってアブラシによる寄生の程度に差がみられた。数百のコムギ品種について、寄生程度の調査を1989年に予備的に行い、選別した8品種について、1990年と1991年にコムギの抵抗性物質とされているDIMBOAの消長を調べた。寄生程度の比較調査は1989年から1992年まで調べ、年と場所による変動を調べた。数千のオオムギの系統間においてはアブラムシ類の寄生数で百倍以上の差があり、抵抗性と感受性の系統を容易に選別できている。しかし、コムギにおいては数十倍の寄生数を示す系統は見出せなかった。系統間に年と場所での抵抗性と感受性の差が見られても、遺伝的であることが確認された系統はなかった。オオムギと比較して、供試した数百のコムギ系統の中には、アブラムシの寄生が非常に多い感受性系統はなく、供試した全てが多少とも抵抗性の形質を示した。コムギの抵抗性物質であるDIMBOAは冬期2月下旬で約1μg/g新鮮葉の含量であり、生育に伴って減少した。減少の程度は系統間で差が余りなかった。換言するとDIMBOAのない、あるいは極端に少量の系統はなく、全ての系統で抵抗性の一因となっていると考察された。コムギにおいては、オオムギで顕著な抵抗性物質と考えられているインドールアルカロイドのグラミンは検出されなかったが、オオムギで検出されなかったDIMBOAが顕著な抵抗性成分として検出され、これによって両ムギの質的な差異が示された。コムギはもう一つの抵抗性物質である水溶性のアコニット酸をオオムギの約10倍も含有しており、コムギではDIMBOAとアコニット酸の両者がアブラムシ抵抗性物質として作用していると考えられた。
kn-abstract=Aphids sometimes severely infest wheat plants, mainly sucking phloem sap and disrupting tissues, and in a few cases act as virus vectors. There are resistant and susceptible varieties of wheat against aphids. DIMBOA (2,4-dihydroxy-7-methoxy-1,4-benzoxazin-3-one) is a substance causing resistance to animals including aphids. The change in the amounts of DIMBOA with growth in eight wheat varieties was estimated in 1990 and 1991. Wheat seeds were sown at mid-November in the former year and harvested in mid-June. Aphids appeared from early April, increased with the growth of the wheat, and finally decreased with the senescence of the wheat. Rhopalosiphum maidis appeared early in the season, R. padi appeared late, Schizaphis graminum and Sitovion akebiae appeared intermittently in the season. S. graminum appeared more frequently on wheat than barley. DIMBOA was detected from wheat but not from barley. Gramine (N,N-dimethyl-indole-3-methanamine) was detected from barley, and is known as an important resistance substance. However gramine could not be detected in wheat. DIMBOA was found in higher amounts in young wheat, and gradually decreased with growth. A clear relationship between the aphid population and DIMBOA amounts could not be observed. However, all the wheat varieties used in this experiment seemed to have resistance against aphids. The resistance was compared with barley susceptible lines. DIMBOA was presumed to share the property of resistance with aconitic acid in wheat.

en-abstract=大麦はアブラムシ類の被害を受けることがある。ウイルス病の伝搬者として、また栄養分の掠奪者と組織細胞の破壊者として、害を及ぼしている。オオムギの系統の中にはアブラムシに対して、抵抗性の系統と感受性の系統がある。インドールアルカロイドであるグラミンはアブラムシを含む各種の動物に対して、毒性を有している。抵抗性と感受性の両者を含む27系統のオオムギについて、幼苗から成熟期迄のグラミン含量の変化を1989、1990、1991年の3年間にわたって調べた。オオムギは前年の11月20日頃播種し、翌年6月上旬に収穫した。全ての系統において幼苗は多量のグラミンを含有していた。抵抗性系統は感受性系統よりも多量に含有していた。この違いは、幼苗期においても成熟期におけるのと同じであった。全系統において、出穂前までの伸長生育期にグラミン含有は漸次減少した。4月初旬の減少し終わった時期に、アブラムシ類は飛来し、繁殖を始めた。4月下旬から5月上旬にアブラムシ類の密度は最高に達した。この時期に近づくに伴い抵抗性系統と感受性系統の明瞭な違いが見られ、グラミン含量の差も明瞭であった。グラミン含量は収穫まで余り変化せず、葉の老熟に伴う水分消失のため、単位重量当たりの換算値では多量になっていた。感受性系統は抵抗性系統に比べて、アブラムシ類が飛来し、繁殖する時期に、グラミンの分解が盛んで、少量のグラミンのみしか検出されなかった。分解に関与する機構が感受性系統では抵抗性系統よりも高いことが、系統間の差の大きな原因と推定された。
kn-abstract=Barley plants may be severely damaged by aphids,mainly because they may transmit viruses,suck food and disrupt tissues. There are resistant lines and susceptible lines in barley to aphids. The changing amounts of gramine from seeding to mature stages were investigated on 27 lines from 1989 to 1991. Barley was sown aound November 20 in the year and harvested in early June. Higher amount of gramine in seeding stage in all lines were detected,then gradually decreased with barley growth until 2 to
3 weeks before the earing stages. At this time aphids had appeared on the barley leaves and increased in population. The susceptible lines had a tendency of greater decrease in gramine content than the resistant lines at the time of aphid population growth. The maximum aphid population was observed from the end of April to early May. The gramine content was almost constant this time. The degradation mechanisms may be important factors determining the defference of resistant and susceptible lines at the aphid population growth stage. The susceptible lines may have a higher activity of degradation mechanism.

en-abstract=禾本科植物の葉に含まれているアコニット酸を抽出定量し、寄生性のアブラムシに対する耐性との関係を調べた。トウモコロシの16系統、典型的な抵抗性と感受性のソルガムの2系統、ヒエ類の3亜種、イネの2系統、オオムギの9系統とコムギの8系統について比較した。一般的に逆齢期の植物は老熟期の植物より高い含量であった。トウモコロシは全体として200μg/g以上を含有する系統が多く、アブラムシに対して耐性を示した。外部からトウモロコシアブラを導入し、そう推移とアコニット酸の関係を調べたところ、比較的耐性を示す系統は多量に含有し、アブラムが寄生し易い系統は少量であった。ヒエノアブラムシに耐性のソルガムの系統は感受性の系統の約2倍のアコニット酵を含量していた。タイヌビエのみがイヌビエ、ヒメタイヌビエ、イネのホウレイとアケボノと異なって、成熟するにつれてアコニット酸の含有量が増加していた。このことはトビイロウンカによって稲田が坪枯れの時にタイヌビエのみが青春としており、それはアコニット酸によるとの報告と一致している。オオムギは比較的少量のアコニット酸が検出され、耐性における作用は弱く、オオムギには特有のグラミンがあり、これがアブラムシに対する耐性の主要因に成っていると考えられた。コムギは少量のアコニットさん含有であり、グラミンは検出されなかった。数個の未同定の作用力不明の多量成分と摂取忌避作用の認められる微量のフェノール酸もあった。いくつかの成分が耐虫性成分であるが、アコニット酸も重要な要因と考えられた。
kn-abstract=Plant-feeding arthropods, in the course of evolution have adapted differentially to their plant host and thereby gained access to the available resource. Plants responded by developing chemicals and morphological defense to arthropods. Some secondary metabolic substances in plants have been assumed to be toxic or deter feeding. Aconitic acid in the barnyard-grass subspecies, Echinochloa oryzycola Vasing has been reported to be an active component against brown rice planthopper Nilapalvata lugens (Stal) (Kim et al. 1976). Aphids, Ropalosiphum padi. L., R. maidis (F.), Schizaphis graminum (Rondani)and Sitobion akeviae (Shinji), are important pests of barley (Hordeum vulgare L.) and wheat (Triticum aesstivum L.). On barley plants, more than seven aphids per stem causes yield loss (Hansen 1990). R. maidis sometimes severely infested corn plants (Zea mays L.), the leaves being wet by honeydew. Melanaphis sacchari (Zehntner) caused brownish decay of leaves of a susceptible line・ of sorghum Sorghum bicolor Moench. We examined the relation between aconitic acid contents and aphid densities in wheat, barley, corn, sorghum, barnyard grass and rice (Rustamani et al. 1992a). The plants containing larger amounts of aconitic acid showed a moderate degree of resistance to aphids. The findings obtained on additional lines in 1991, and on other components, such as DIMBOA (2,4-dihydroxy-7-methoxy-1,4-benzoxazin-3-one), are reported herein.
