The evaluation procedures could be divided into tow groups : procedures by clinical observations and procedures by findings relating to action mechanisms of spa therapy. Clinical evaluations have been performed by subjective and objective symptoms, the degree of reduction of drugs used for treatment of bronchial asthma, and improvement of each parameter in ventilatory function test. The results obtained by these procedures are frequently reported as clinical observations. On the other hand, evaluations based on the findings relating to action mechanisms of spa therapy have been carried out as follows : 1) observations for immuno-allergic reactions which play an important role in pathogenic mechanisms of bronchial asthma. 2) observations for autonomic nerve system, and 3) for endocrine glands, which have a close correlation with onset mechanisms of bronchial asthma. In the present article, the effects of environmental climate around the hot springs on bronchial asthma were discussed according to the reports published. Mountain climate, sea climate and underground climate are reported to be effective for the therapy of bronchial asthma. Although these climates have shown remarkable effectiveness on bronchial asthma, each of them has climatic characteristics in treatment for bronchial asthma. Mountain climate has low content of oxygen and low vapor pressure, and patients with bronchial asthma require several weeks to adapt themselves for the climate. Sea climate is rich in cations and anions, and Ca(2+) and Mg(2+) included in sea water aerosol act beneficially on mucous membrane of airways in patiepts with bronchial asthma. Underground climate includes a lot of anions, which activate the activity of mucous membrane and ability of clearance of airways. Tissue perfusion was evaluated before and after CO(2) treatment on the patients with connective tissue disorders by means of medical mass spectrometer. CO(2) treatment was performed in 2 ways ; one was with CO(2)-cabine, 20 litres per minute at 40℃ for 20 minutes, and the other was with an artificial CO(2) bathing prepared with 2 tablets of BUB-KAO, a 50 g sodium hydrogencarbonate and succinic acid tablet, in 280-litre-tub at 40℃ for 20 minutes. Case 1, 33-year-old female, suffering from chronic rheumatic arthritis with rheumatic thromboangiitis obliterans in the left 5th toe. Main arterial trees were found intact arteriographically. Her joint pain and muscle stiffness have been improved markedly after CO(2) treatment with cabin every day for one month though every medicine for rheumatism made little effect. Case 2, 46-year-old female, suffered from progressive systemic scleroderma. Major blood supply was found not blocked in upper extremities arteriographically. Tissue perfusion was found poor in muscle tissue of each forearm before CO(2) treatment with cabin, whereas an improved tissue perfusion was confirmed after one month of serial CO(2) treatment, leading to ameliorate persistent muscle stiffness, though every medicine for her made slow effect. Case 3, 48-year-old female, suffering from the weakness in strength with no positive laboratory findings of connective tissue disorders. Tissue perfusion has been found quite improved following serial artificial CO(2) bathings for 12 months, being able to work hard every day as a nurse. In this method, 2.0mℓ of water sample was directly mixed with 15 mℓ of Hionic-Fluor in a vial, which was then filled up with additional Hionic-Fluor ; 190 minutes were allowed for the radioactivity to equilibrate. The radioactiviity of Radon was measured by the Liquid Scintillation Counter (Tri-Carb 460C). Radon content was easily calculated with the proposed formula by an electric desk calculater. The reproducibility was proved CV=5.77 % ; the lowest detection limit was 0.2 nCi/l. Patients were randomly divided into two groups : eight patients into a control group (continuance of spa-bathing) and 10 patients into a spa-drink therapy group (continuance of spa-bathing plus commencement of spa-drink therapy). Spa-drink therapy refers to drinking 200mℓ of warm spa-water (42℃) two times a day : between meals in the morning and in the afternoon. Exocrine pancreatic function was evaluated by fecal chymotrypsin activity, using a colorimetric method (Boehringer-Mannheim). Following results were obtained : 1) With 2-week's spa-drink therapy, fecal chymotrypsin activity was significantly enhanced in 40 % of the patients, while none of the controls showed an increase in fecal chymotrypsin activity. 2) Fecal chymotrypsin activity at the time of 4 weeks was similar to that obtained at the time of 2 weeks in both groups. Terfenadine did not inhibit the release of histamine from basophils of healthy subjects induced by anti-IgE, and also of patients with bronchial asthma induced by house dust when the in vitro examination was performed by a whole blood method. 2. Histamine release from basophils of patients with bronchial asthma was significantly inhibited when washed leucocytes was preincubated with terfenadine in vitro. The release of LT B(4) from leucocytes was also inhibited. 3. In vivo examination of inhibitory action of terfenadine showed no effect on basophil histamine release of healthy subjects by anti-IgE and of asthmatic subjects by house dust, although skin test by house dust in these subjects was inhibited 60 min after administration in vivo. Thirty-four subjects were randomly divided into three groups : 10 subjects for warm spa-water (40±C) infusion group, 10 subjects for warm tap-water infusion group, and 14 subjects for warm air infusion group. Intragastric infusion of water or air was performed through the fiberscopic infusion channel. Gastric mucosal blood flow was determined just before and 10 min after the infusion on the three points of gastric mucosa : lesser curvature of the angle, the antrum and the pylorus. Following results were obtained : 1) Spa-water exerted a beneficial effect on gastric mucosal blood flow as compared to air. The difference was statistically significant on all of the three points. 2) Spa-water tended to exert a beneficial effect on gastric mucosal blood flow as compared to tap-water. The difference, however, was significant only on the mucosa of the gastric antrum. In conclusion, it was suggested that spa-drink therapy was useful for the treatment of chronic gastritis and gastric ulcer in which impairment of gastric mucosal blood flow plays an important pathogenetic role.
